  • Objectives
    Suppression head impulse paradigm (SHIMP) in video head impulse test is now newly used to test vestibular function. The aim of this study was to analyze normative values of SHIMP for healthy Korean subjects in each decade of life.
  • Methods
    SHIMP and HIMP responses were measured with the video head impulse test in 70 healthy subjects. Vestibulo-ocular reflex gain and anticompensatory saccade were analyzed and compared at each decade of life.
  • Results
    All subjects produced anticompensatory saccades in SHIMP. Gain values did not vary significantly with age. Gain values in SHIMP were lower than gain values in HIMP. The gain values of rightward impulse were higher than the gain values of leftward impulse.
  • Conclusions
    Gain values and anticompensatory saccades in SHIMP were consistently equal in each decade of life. Normative values of SHIMP seems largely unaffected by aging.
비디오두부충동검사는 세반고리관에 의한 전정안반사의 이득을 측정할 수 있으며 온도안진검사 및 회전의자검사에 비하여 일상생활에서 실제 머리 움직임을 나타내는 주파수 자극에 의한 검사를 할 수 있어 널리 사용되고 있다[1]. 비디오두부충동검사에서 기존의 head impulse paradigm (HIMP) 검사 외에 다른 패러다임을 적용한 suppression HIMP (SHIMP) 검사는 2016년 MacDougall 등[2]에 의해 소개되었으며 HIMP와 달리 고정된 목표물을 주시하지 않고 머리와 함께 움직이는 목표물을 주시하며 검사를 시행함으로 정상인에서는 전정안반사로 이전의 목표물을 주시하다가 머리의 회전과 함께 이동하는 목표물을 따라잡기 위해 머리의 회전과 같은 방향으로 반보상적 단속운동(anticompensatory saccade)이 나타난다(Fig. 1). SHIMP 검사의 중요한 지표는 이득(gain)과 반보상적 단속운동이며 이는 잔존 전정기능을 나타내고 HIMP 검사와 상호 보완적으로 결과 해석에 적용할 수 있다[3]. 정상인에서 HIMP 검사의 이득은 연령별 차이가 없는 것으로 보고되고 있으며[4] SHIMP 검사의 이득은 HIMP 검사에 비하여 낮은 것으로 보고되고 있으나[2,5] 아직까지 SHIMP 검사 결과의 연령별 변화에 대해서는 제시되지 않고 있다. 이 연구에서는 정상 한국인에서 비디오두부충동검사의 새로운 패러다임인 SHIMP 검사의 결과에 대하여 연령별 변화를 분석하고 HIMP 검사와의 차이를 확인하고자 하였다.
귀 질환을 가지지 않은 정상인 중 검사에 동의한 10대, 20대, 30대, 40대, 50대, 60대, 70대 각각 10명씩 70명을 대상으로 하였다. 이 연구는 병원 임상연구 심의위원회의 승인(2018-12-003)을 얻었으며 검사 전 문진을 통하여 과거력, 약물력, 수술력 등을 조사하였고 안구질환 또는 경부질환이 있거나 어지럼, 이과 수술의 과거력이 있는 경우, 검사 시행을 위한 교정단계(calibration)가 적절히 이루어지지 않은 경우, 검사 시행에 대한 이해가 어려워 검사를 잘 수행하지 못하는 경우는 연구에서 제외하였다. 검사는 ICS Impulse 기기(GN Otometrics, Taastrup, Denmark)를 이용하여 1미터 거리에서 목표점을 주시하고 회전 각도 15°–20°, 최대 각속도 150°–250°/sec로 측반고리관의 HIMP 검사와 SHIMP 검사를 양방향으로 각각 20회 반복 시행하였다.
검사를 시행 후 전정안반사 이득과 머리의 회전 자극이 시작된 직후부터 반보상적 단속운동이 시작되는 시점 사이의 시간 간격인 지연값(latency), 각 단속운동 속도의 절대값들을 합하여 총 자극 횟수로 나눈 최대 단속운동 속도(peak saccade velocity) 및 이득 비대칭(asymmetry; [1−Gi/Gc]×100) 지표를 평가에 이용하였다. 통계학적 분석은 IBM SPSS Statistics ver. 20.0 (IBM Co., Armonk, NY, USA)을 사용하였으며 HIMP 검사와 SHIMP 검사의 이득 비교는 Wilcoxon signed rank test를 이용하여 분석하였고 이득, 지연값, 최대 단속운동 속도, 이득 비대칭의 연령 군별 비교는 Kruskal-Wallis test를 시행하였으며 두 군 간의 비교는 Student t-test를 이용하였다. p 값 0.05 미만일 때 통계학적으로 유의하다고 판단하였다.
1. 이득
HIMP 검사 시 전체 피검자의 이득은 우측 측반고리관 1.02±0.08, 좌측 측반고리관 0.95±0.08이었으며 SHIMP 검사 시 전체 피검자의 이득은 우측 0.90±0.09, 좌측 0.87±0.08이었다. HIMP 검사와 SHIMP 검사 모두 우측의 이득이 좌측보다 유의하게 높게 관찰되었다(p<0.01) (Fig. 2). HIMP 검사의 연령별 이득은 10대, 20대, 30대, 40대, 50대, 60대, 70대 각각 우측 1.01±0.04, 0.97±0.07, 1.00±0.07, 1.05±0.07, 1.02±0.08, 1.05±0.09, 1.01±0.11, 좌측 0.96±0.02, 0.92±0.04, 0.95±0.04, 0.97±0.09, 0.94±0.07, 0.98±0.11, 0.93±0.11이었고 연령별 군들 간의 유의한 차이는 관찰되지 않았다. SHIMP 검사의 연령별 이득은 10대, 20대, 30대, 40대, 50대, 60대, 70대 각각 우측 0.95±0.06, 0.87±0.05, 0.87±0.07, 0.91±0.09, 0.86±0.08, 0.96±0.09, 0.91±0.11, 좌측 0.89±0.04, 0.83±0.03, 0.82±0.09, 0.89±0.09, 0.83±0.07, 0.92±0.09, 0.88±0.08이었고 연령별 군들 간의 유의한 차이는 관찰되지 않았다. 전체 피검자의 HIMP 검사와 SHIMP 검사 간 이득의 차이는 우측 0.11±0.08, 좌측 0.08±0.06으로 유의한 차이를 보였으며 각각의 연령군에서도 HIMP 검사와 SHIMP 검사의 이득은 유의한 차이를 보였다(p<0.05) (Figs. 3, 4).
전체 피검자의 HIMP 검사 이득 비대칭은 6.68%±4.25%이었으며 SHIMP 검사의 이득 비대칭은 5.62%±4.49%이었고 두 검사 간의 유의한 차이는 관찰되지 않았다(Fig. 5). SHIMP 검사의 연령별 이득 비대칭은 10대, 20대, 30대, 40대, 50대, 60대, 70대 각각 7.4%±3.5% 4.2%±4.4%, 6.2%±5.4%, 3.3%±3.1%, 5.5%±4.3%, 6.5%±5.6%, 6.3%±4.3%로 연령별 유의한 차이는 관찰되지 않았다.
2. 반보상적 단속운동
SHIMP 검사 시 반보상적 단속운동의 발현은 모든 피검자에서 발생하였으며 전체 피검자의 지연값은 우측 192.3±43.8 msec, 좌측 202.5±36.8 msec였고 최대 단속운동 속도는 우측 306.2°±44.3°/sec, 좌측 289.2°±58.7°/sec이었다. 우측 검사 시 좌측보다 지연값은 유의하게 짧았으며 최대 단속운동 속도는 높게 관찰되었다(p<0.05) (Fig. 6). 반보상적 단속운동의 지연값 및 최대운동 속도를 연령별로 분석한 결과 유의한 차이는 관찰되지 않았다(Table 1).
비디오두부충동검사는 생리적 두부 운동에 의한 반고리관에서 발생하는 전정안반사를 검사하는 것으로서 머리 회전 각속도와 안구 움직임의 각속도를 표시하게 되며 신속안구운동 유무를 확인할 수 있다. 검사 결과의 두 가지 중요한 지표는 전정안반사 이득과 신속안구운동이며 HIMP와 SHIMP 두 가지 패러다임 모두 이득을 측정할 수 있고 HIMP의 재고정신속안구운동은 전정기능 손상을 나타내는 반면 SHIMP의 반보상적 단속운동은 전정기능을 나타내며 전정기능소실이 있는 환자에서 잔존 전정기능을 확인할 수 있어 전정재활을 확인하는데 유용하게 사용할 수 있다[2,6].
SHIMP 검사의 이득은 HIMP 검사에서 나타날 수 있는 숨은신속안구운동(covert saccades)이 나타나지 않으므로 HIMP 검사의 이득에 비하여 더 신뢰할 수 있는 것으로 알려져 있으며 HIMP 검사의 이득에 비하여 0.06 정도 낮게 측정되는 것으로 보고되었다[2,7]. 이 연구에서도 전 연령에서 SHIMP 검사의 이득이 HIMP보다 0.08–0.11 정도 유의하게 낮게 측정되었으며 이는 SHIMP 검사 시 전정안반사를 억제하는 안구 움직임을 측정하여 상대적으로 낮은 것으로 생각한다[5]. 연령에 따른 HIMP 검사의 이득 변화는 차이가 없다는 보고가 많았으나[4,8] 70세 이상의 고령에서 감소한다는 보고도 있다[9]. 이 연구에서 연령에 따른 SHIMP 검사의 이득 변화는 유의하지 않은 것으로 나타났으며 노화에 따른 감각기관의 조직학적 퇴행과는 다른 결과를 보여줬다. 이는 노화에 따라 전정수용기 세포와 신경의 감소가 발생하더라도 소뇌에 의한 조절에 의해 기능적인 전정안반사는 상대적으로 유지되기 때문에 SHIMP 검사의 이득이 유지되는 것으로 생각한다[10].
이 연구에서는 SHIMP 검사의 우측 이득이 좌측보다 유의하게 높은 것으로 나타났으며 Devantier 등[11]이 보고했던 결과와 유사하다. 이는 HIMP 검사에서 우측으로 검사하였을 때 이득이 높게 측정되는 것과 같은 결과이며 검사기기의 카메라가 우측에 있어서 우측으로 검사를 시행할 때 더 큰 회전 자극을 발생하는 것이 원인으로 생각한다. 이외에도 내직근이 외직근에 비해 강한 안구운동을 유발하는 안구 움직임의 생리적인 차이에서 기인하거나 또는 검사자가 오른손잡이여서 나타나는 결과일 수 있다[4,9,12].
SHIMP 검사 시 나타나는 반보상적 단속운동은 전정기능을 대변하며 자발안진과 반대 방향으로 나타나기 때문에 더 쉽게 관찰할 수 있고 지연값 등의 지표를 보다 정확하게 계산할 수 있다[3]. 우측 SHIMP 검사 시 좌측보다 상대적으로 높은 이득은 반보상적 단속운동에도 영향을 미칠 수 있으며 우측의 반보상적 단속운동이 좌측에 비하여 지연값은 짧고 최고 단속운동 속도는 높은 결과를 나타냈다. 연령별로 지연값과 최대 단속운동 속도의 차이가 있는지 분석한 결과 변화가 없는 것으로 확인되었으며 이와 같은 결과 역시 노화가 진행되어도 전정안반사가 상대적으로 유지되기 때문인 것으로 생각한다[10].
HIMP 검사에서 양측의 비대칭은 20% 이내를 정상으로 볼 수 있으며 이득이 정상과 비정상의 경계상에 있을 때 양측의 대칭 여부가 검사 결과를 판독하는 데 도움을 줄 수 있다[13]. 이 연구에서 SHIMP 검사의 이득 비대칭은 HIMP 검사와 차이가 없어 HIMP 검사와 동일하게 결과 판독에 적용할 수 있을 것으로 생각한다.
SHIMP 검사의 중요한 지표인 이득과 반보상적 단속운동은 연령의 변화에 따라 유의한 차이가 없었으며 이는 노화에 의한 영향이 적은 것으로 생각된다. 하지만, 이 연구의 연구 대상자 수가 적으며 80대 이상의 고령층이 포함되지 않아 추후 더 많은 대상자를 포함한 추가 연구가 필요할 것으로 생각한다.
