주기교대안진을 보인 메니에르병 환자 1예

Trans Abstract

Periodic alternating nystagmus (PAN) is a spontaneous horizontal jerky nystagmus that reverses its direction periodically with a quiescent interval. PAN has been reported in acquired and congenital forms. The main lesion site of the acquired form of PAN has been attributed to the caudal brainstem or cerebellum. Herein we report a 63-year-old male patient with Meniere’s disease, who presented PAN during a vertigo attack. The patient demonstrated no abnormality on neurologic evaluation and brain imaging, which is different feature compared to the central or congenital form of PAN. It should be kept in mind that peripheral vestibular disorders such as Meniere’s disease can produce PAN.

서 론

주기교대안진(periodic alternating nystagmus, PAN)은 1분에서 6분 정도의 주기로 한 방향으로 수평안진이 지속되다가 4초에서 20초 정도의 정지기를 가지고, 이후 방향이 반대로 바뀌는 안진이다[1]. 주기교대안진은 Arnold-Chiari 기형과 같은 선천성 질환이나 후천적으로 뇌간과 소뇌를 포함하는 염증, 종괴, 뇌경색 등의 중추성 질환에서 보고되었다[2,3]. 하지만 말초성 질환에 의해 나타난 경우는 흔치 않으며, 세계적으로 보고된 증례도 매우 드물다[4]. 이에 저자들은 메니에르병 환자에서 주기교대안진을 경험하여, 말초성 주기교대안진에 대한 문헌 고찰과 함께 보고하는 바이다.

증 례

63세 남자 환자가 내원 3일 전부터 발생한 어지럼을 주소로 외래를 방문하였다. 환자는 명확한 메니에르병으로 본원에서 정기적으로 경과 관찰하며 세 차례의 어지럼 발작이 있던 환자로, 과거력상 고혈압과 고지혈증으로 약물 치료 중이었다. 환자는 내원 2개월 전, 내원 수 시간 전부터 발생한 어지럼으로 내원하였고 당시 좌측 이명 및 청력 감소를 동반하였다. 당시 비디오안진검사에서 우측으로 향하는 수평 자발안진과 두진 후 안진검사에서 우측으로 안진이 증가하는 소견이 보였고, 순음청력검사에서 우측은 기도청력역치 22 dB, 좌측은 기도청력역치 31 dB로 좌측의 감각신경성난청이 관찰되었다(Figure 1A). 환자가 어지럼으로 재내원했을 때, 이전에 호소하던 좌측 이명 및 청력 감소는 없었다. 이학적 검사상 양측 고막은 정상이었으며, 당일 시행한 청력검사에서 우측은 22 dB, 좌측은 24 dB로 이전 검사보다 호전된 결과를 보였다(Figure 1B). 환자는 비디오안진검사에서 좌측으로 향하는 수평성 자발 안진이 1분간 나타나다가, 12초간의 정지기 이후 방향이 바뀌어 우측으로 향하는 수평 안진이 2분 동안 관찰되었다. 이후 다시 5초간의 정지기를 가진 후, 다시 좌측으로 향하는 수평 안진이 관찰되었고, 비디오안진검사로 이를 확인하였다(Figure 2). 기존 3번의 어지럼 발작 시에 관찰되었던 안진과는 다른 양상이 확인되어, 환자는 자세한 신경학적 진찰을 시행하였으나 중추신경계 이상을 의심할만한 소견은 관찰되지 않았다. 환자는 전형적인 메니에르병의 급성기에 보이는 청각 증상이 없었으며 청각검사 소견도 정상으로 확인되어 중추성 원인을 감별하기 위해 입원하였다. 어지럼의 중추성 원인 감별 위해 시행한 뇌 자기공명영상에서 뇌간 및 소뇌에 이상 소견은 관찰되지 않았다(Figure 3). 이후, 환자는 메니에르병의 급성 악화로 진단되어 증상 조절을 위해 약물치료(Dimenhydrate 15 mg/day, Betahistine mesylate 36 mg/day, Isosorbide 90 mL/day)를 시행하였다. 입원 2일 째, 환자가 느끼는 어지럼은 전날보다 호전되었다. 비디오안진검사에서 입원 당일에 관찰된 주기교대안진은 더 이상 나타나지 않았으며, 우측으로 2.71 deg/sec의 수평성 자발 안진만 관찰되었다. 전기와우도검사에서 summating potential/action potential ratio는 좌측 0.46, 우측 0.35로 좌측에서 정상치보다 높은 수치를 보였으며, 전정근유발전위(vestibular evoked myogenic potentials)는 정상이었다. 입원 3일 째, 환자의 증상은 모두 호전되었으며, 비디오안진검사에서 자발안진 및 두진후 안진은 더 이상 관찰되지 않았다. 같은 날 시행한 냉온교대 온도안진검사에서 좌측 28%의 반고리관마비, 우측 11%의 방향우위성을 보였으며, 환자는 증상이 호전되어 퇴원하였다.

고 찰

본 증례는 좌측 메니에르병으로 경과관찰 중이던 환자에서 기존의 메니에르병 어지럼 발작과 달리, 청각 증상없이 어지럼이 심해짐과 동시에 주기교대안진이 나타난 경우이다. 환자의 자기공명영상에서 뇌간 및 소뇌에 이상소견이 없었으며 안진을 유발할 만한 약물 복용력도 없었다. 기존에 보인 말초성 주기교대안진의 경우 안진의 주기가 불규칙하고 지속시간이 짧은 특징을 보인 반면[4], 본 증례의 경우 안진의 주기가 규칙적이었고 지속시간도 수 분에 걸쳐 나타나 중추성 주기교대안진과 감별이 어려운 아주 드문 증례이다.

주기교대안진은 선천적으로 발생하기도 하고, 뇌간과 소뇌를 포함하는 염증, 종괴, 뇌경색 등의 중추성 병변에서 보고되었다[2,3]. 드물게 진주종, 만성 중이염, 작은 청신경 종양, 메니에르병 등의 말초성 전정질환에서도 보고된 바 있다[5,6].

선천성으로 발생하는 주기교대안진의 경우 방향의 역전 주기가 덜 규칙적이고, 속도증가형파형, 시운동성안진의 역전 등의 특징을 보일 수 있다[2]. 또, 소뇌의 소절과 목젖이 선택적으로 제거된 원숭이가 암실 상황에서 주기교대안진을 보였고[7] 최근 인간에서도 소뇌의 소절 경색 이후 주기교대안진이 보고된 바 있다[8]. 따라서, 중추성 주기교대안진의 경우 소뇌의 소절, 목젖 혹은 그와 연결된 전정신경핵이나 뇌간구조의 이상으로 발생하여 속도저장기능이 억제가 안되어 연장된 안진이 나타나며, 이어서 중추성 보상기전에 의해 안진의 방향이 역전되고 이러한 주기가 반복되어 주기성 교대안진이 발생하는 것으로 설명하고 있다[9].

주기교대안진이 말초성 전정질환에서 발생하는 경우는 상당히 드물며, 1920년 Borries [10]에 의해 미로 누공 환자에서 주기교대안진이 처음 보고된 이후로 매우 적은 증례들이 보고되었다[4,11]. 말초성 병변에 의해 발생한 주기교대안진의 기전은 중추성 교대안진과는 다를 것이라고 생각된다. 기존에는 말초성 질환에서 주기교대안진이 나타나는 이유를 현대의학으로 발견하지 못하는 중추성 병리 소견에 의해 발생한다고 이해하였다[4]. 하지만 최근 몇몇 연구에서 말초성 주기교대안진의 병리 기전을 제시하였으며, 그 기전은 질환의 초기 자극기 시 병변측에 있는 전정핵이 자극을 받아 병변측으로 뛰는 안진이 발생하고, 마비기로 변해가게 됨에 따라 전정핵으로 들어가는 자극이 줄어들어 안진의 방향이 병변의 반대측으로 바뀌게 된다고 제시하고 있다. 주기가 지속되는 이유는 질환의 자극기와 마비기가 반복되면서 주기교대안진을 보이게 되고, 그 사이에 이행기가 있어 안진이 멈추는 시간이 나타난다는 것으로 추정하고 있으나 그 정확한 기전은 아직 모른다[4,12,13].

최근 말초성 주기교대안진에 대한 한 연구에서 말초성 전정질환과 중추성 전정질환에서 보이는 주기교대안진의 감별진단을 정리하였는데, 그 결과는 다음과 같다. 첫째, 말초성 전정질환에서 보이는 주기교대안진은 발생 48시간 이내에 일측으로 향하는 자발안진으로 바뀌게 되며, 둘째, 시운동성 안진이나 원활추종운동 등의 수행에 있어 문제가 없고, 셋째, 교대안진의 주기가 덜 규칙적이며, 시고정이 없을 때만 보이는 특징이 있다. 마지막으로, 신경학적 검사에서 이상 소견이 보이지 않으며 달팽이관의 이상을 시사하는 청력 감소, 이명 등이 동반된다[14].

저자들은 말초성 질환에서 관찰하기 어려운 주기교대안진을 메니에르병 환자에서 관찰하였기에 증례를 보고하며, 중이염이나 미로누공과 같은 말초성 질환에서도 주기교대안진이 나타날 수 있어, 반드시 중추성 주기교대안진과 감별해야 함을 강조하는 바이다.
