  • Rotatory vertebral artery syndrome (RVAS) is characterized by recurrent attacks of vertigo, nystagmus, and syncope induced by compression of the vertebral artery during head rotation. A 60-year-old man with atlas vertebrae fracture presented recurrent attacks of positional vertigo. Left-beat, upbeat and count clock-wise torsional nystagmus occurred after lying down and bilateral head roll (HR) showing no latency or fatigue. Magnetic resonance imaging revealed foramen magnum stenosis (FMS) and dominancy of right vertebral artery (VA). The flow of the right VA on transcranial Doppler decreased significantly during left HR. The slower the velocity was, the more the nystagmus was aggravated. RVAS can be evoked by FMS causing compression of the VA. And the nystagmus might be aggravated according to the blood flow insufficiency.
회전성척추동맥증후군(rotational vertebral artery syndrome)은 머리의 회전성 움직임으로 인하여 척추동맥의 협착이나 폐색이 유발되고 그로 인하여 뇌간, 특히 교뇌-연수(pons-medulla) 부위의 허혈로 인한 어지럼을 가리킨다. 이 때 유발되는 안진은 다양한 양상으로 나타나지만 주로는 수평-회전-수직 안진이 혼재된 양상으로 나타나는 것으로 알려져 있다[1]. 안진이 발생할 때 환자들은 어지럼을 느끼는 경우도 있지만 어지럼 없이 안진만 나타나는 경우도 있다[2]. 저자들은 1번 경추골에 외상으로 인한 변형이 있는 환자가 보이는 중추성자세성안진을 경험하였기에 보고하고자 한다.
60세 남자가 두 달 전부터 발생하는 어지럼으로 내원하였다. 환자는 3달 전부터 고혈압으로 약물을 복용하였다. 어지럼은 빙글빙글 도는 양상이었으며, 누운 자세를 취할 때마다 발생하였다. 특히 누워서 고개를 돌릴 때 어지럼이 심하다고 하였다. 걷거나 앉아 있을 때 어지럼이 유발되지는 않았으며 이명이나 이 충만감, 청력 저하는 없었다. 환자는 10살 때 높은 곳에서 떨어져 목을 다친 이후로 경추골 이상이 있었다고 들었으나 큰 불편함 없이 지냈다. 신경학적 검사에서 동공 반사는 정상적이었고, 안구 운동의 제한은 없었다. 얼굴 근육 및 얼굴 감각의 이상도 보이지 않았다. 팔다리 위약과 감각 이상은 없었으며, 실조 증상도 나타나지 않았다. 환자의 건반사는 양측 모두 정상적이었다. 비디오안진검사(video-oculography)에서 자발안진 및 주시유발안진은 없었다. 두부충동검사 및 온도안진검사는 모두 정상범위였다. 환자는 앙와위로 누웠을 때, 앙와위에서 고개를 양측으로 돌릴 때(supine rolling test) 좌측, 상방 그리고 반시계 방향의 안진(left-beat, upbeat and count clockwise torsional nystagmus)이 유발되었다. 자세를 취할 때마다 어지럼 증상도 유발되었으나 이명이 동반되지는 않았다. 안진은 잠복기가 없었으며 자세를 바꿀 때까지 지속적이었고, 시간이 지나거나 자세를 반복할 때도 피로를 나타내지 않았다. 두부 magnetic resonance imaging에서 1번 경추골(atlas)의 기형으로 인하여 대후두공(foramen magnum) 협착이 확인되었다(Fig. 1A, B). 또한 좌측 동맥의 혈액의 흐름은 불충분하였고, 상대적으로 우측 척추동맥이 우세하였다(Fig. 1C). 좌측 내경 동맥의 심한 협착이 관찰되었다. 뇌혈관 조영술에서 좌측 척추동맥은 입구부터 혈관이 보이지 않았고, 우측 척추동맥이 좌측 소뇌 영역까지 포함하여 혈류를 공급하고 있었다. 누운 상태에서 환자의 고개를 90도로 회전시켰을 때 우측 척추동맥의 협착이나 막힘은 보이지 않았다. 안진유발 여부는 확인하지 못하였다. 경두개도플러초음파에서 자세를 취할 때 우측 척추동맥의 흐름을 관찰하였다. 환자는 누울 때, 그리고 양측으로 고개를 돌릴 때 혈액의 흐름이 감소하였다(Fig. 1DI) 특히 우측으로 고개를 돌릴 때 의미 있게 감소하는 것이 보였다(Fig. 1H, I) 혈류의 속도와 안진의 속도를 비교했을 때, 혈류의 속도가 감소할수록 안진의 속도도 감소하는 것을 확인할 수 있었다(Fig. 1DI). 또한 환자는 우측으로 고개를 돌렸을 때, 즉, 혈액의 속도가 가장 느렸을 때, 어지럼을 크게 호소하였다. 환자는 뇌혈관 협착에 대한 약물 치료로 항혈전제(aspirin 100 mg, clopidogrel 75 mg) 및 아토바스타틴 80 mg을 복용하였다. 환자는 혈압약을 복용 후 어지럼이 유발된다고 하여 혈압약은 중단하였고, 이후로 가끔 걸을 때 순간적인 어지럼만 있다고 하였으며 이전의 자세성 어지럼은 호전되었다고 하였다. 3개월 뒤에 확인한 비디오안진검사에서 양측으로 고개를 돌릴 때, 좌측 수평 및 하방안진이 경미하게 관찰되었으나 어지럼을 호소하지는 않았다.
중추성안진은 그 발생 원인이 다양하나 그 중에서도 회전성척추동맥폐색(rotatory vertebral artery occlusion)에서 관찰될 수 있다[3,4]. 이러한 안진의 발생 기전은 전정신경전달경로의 일시적인 허혈로 인한 것으로 제시되어 왔다[1]. 지금까지 보고된 회전성척추동맥폐색에서 관찰된 안진은 주로 수평-회선-수직이 혼재된 양상(mixed horizontal-torsional-vertical nystagmus)이었으나 수직 안진이 주된 양상이었으며 하방 안진이 대부분이었다[5]. 이는 내이에서 앞-수평 반고리관을 담당하는 앞미로동맥의 일시적인 허혈로 인한 비대칭 자극(asymmetric excitation) 으로 설명하고 있다[5]. 상방안진이 관찰된 경우가 보고된 적도 있었지만 이 경우는 순수상방안진만 나타났었다[6].
이 증례의 경우, 수직-회선 안진의 복합성 안진으로 관찰되고 상방안진과 함께 수평안진이 좌측으로(눌린 동맥의 반대방향) 향한 것으로 미루어 볼 때, 우측 내이의 앞반고리관과 수평반고기관의 억제가 그 기전으로 설명될 수 있다. 이것은 기존에 보고된 일시적 허혈로 인한 자극(excitation)이 아닌 기능의 억제(inhibition)로 나타났다는 점에서 차이가 있다고 하겠다.
이번 환자에서 나타나는 안진의 경우는 경추기형으로 인한 대후두공협착이 교뇌-연수 근처의 뇌간을 직접적으로 압박하여 발생한 것인지, 척추 동맥의 압박으로 인한 간접적인 뇌허혈의 결과인지는 불분명하다. 그러나 비침습적인 검사 방법인 경두개초음파를 이용해서 혈액의 흐름의 변화를 확인하였고 그에 따른 안진 변화와 환자의 증상이 연관성이 있음을 간접적으로 보여주었다. 뇌혈관 조영술이 척추동맥의 폐색을 확인할 수 있는 최적의 검사일 수 있으나 침습적 검사이고, 폐색이 아닌 혈액의 속도만 떨어지는 경우는 정량적으로 확인하기가 어려울 수 있다. 이 환자의 경우에도 뇌혈관 조영술을 하였으나 척추동맥의 폐색은 확인할 수 없었다.
환자의 경추 기형은 오래전부터 있었으나 어지럼은 이번에 처음 발생하였다는 것은 다른 요소가 개입되어 있음을 시사한다. 환자는 평소에는 수축기 혈압이 160대로 매우 높은 상태였으나 최근 들어 혈압약을 복용하면서 혈압이 급격히 하강한 상태였다. 혈압약을 중단한 이후에 어지럼 및 안진이 좋아진 것은 급격한 혈압조절로 인한 뇌혈류 감소로 뇌허혈에 취약한 조건에서 중추성안진과 어지럼이 발생할 수 있었음을 유추해 볼 수 있겠다.
