Abstract

Objective
The purpose of this study was to identify the influence of sleep position on benign paroxysmal positional vertigo (BPPV).
Methods
Four hundred sixty patients diagnosed as posterior or horizontal canal BPPV were analyzed retrospectively. All patients were asked about their preferred sleep positions among the following four choices: supine, right or left lateral, or no predominant side via questionnaire at initial visit and after 1month. Patients were classified into two groups: affected side group meaning that the patient preferred to sleep ipsilateral to the affected ear and other position group including all positions other than lying lateral to the affected side after treatment. We analyzed the change in the sleep pattern after treatment and compared the recurrence rate between the two groups.
Results
Our study included 244 patients with posterior canal BPPV (PC-BPPV) and 216 patients with horizontal canal BPPV (HC-BPPV). Statistically significant correlation was demonstrated between sleep position side and the affected side by BPPV. The number of patients who slept on the affected side by BPPV decreased, while the number of patients who slept on the healthy side increased significantly after treatment. There was no statistically significant difference in the recurrence rate between the two groups.
Conclusion
There was significant correlation between the sleep position side and the affected side in PC-BPPV and HC-BPPV. The patient had a tendency to avoid lying lateral to the affected side by BPPV during sleep after treatment, however the change in sleep position did not influence the recurrence rate of BPPV.

서 론

양성 돌발성 두위현훈(benign paroxysmal positional vertigo, BPPV)은 자세 변화에 따라 반복적으로 나타나는 회전성 어지럼을 특징으로 하는 가장 흔한 재발성 말초성 전정질환이다[1]. 병태생리학적 소견으로는 난형낭반에 위치한 이석이 탈락되어 반고리관 내에 유리결석 형태로 들어가 피스톤 역할을 함으로써 팽대부릉정이 움직이는 반고리관결석설과 반고리관의 팽대부릉정에 결석이 붙음으로써 중력 방향에 따라 팽대부릉정이 움직이게 되는 팽대부릉정결석설이 있다[2,3].

BPPV는 확인될 수 있는 원인의 유무에 따라 일차성과 이차성으로 나뉘며, 이차성 원인으로는 두부 외상(8.5%), 전정신경염(0.8%), 메니에르병(0.6%) 등이 있으나, 대부분의 경우 원인이 알려져 있지 않은 일차성(70%내외)이다[1,4,5]. BPPV는 수면 전후 자세 변화와 함께 증상이 시작되는 경우가 많고, 특히 수면 후 아침에 침대에서 일어날 때 자주 증상이 발생하는데, 이를 통해 수면이 BPPV의 발생에 영향을 줄 수 있다는 추정이 가능하다[6]. 실제로 최근 수면 중 머리 방향 자세와 이환되는 반고리관의 방향 간에 상관관계가 있다는 연구들이 발표되고 있다[7-9]. 그러나 대부분의 관련 연구들이 제한된 수의 적은 표본 수를 대상으로 하거나 후반고리관 환자들만을 대상으로 하고 있고, 수면 자세가 BPPV의 재발률에 미치는 영향에 대해서도 연구자간에 상반되는 결과를 보이기도 한다[10-12]. 이에 저자들은 후반고리관 뿐만 아니라 수평반고리관 BPPV 환자들의 수면자세와 BPPV 발생과의 관련성을 확인하고, 첫 진단 후 수면 자세의 변화 양상에 따른 BPPV 재발 여부를 알아보고자 하였다.

대상 및 방법

2012년 1월부터 2014년 12월까지 본원 이비인후과로 내원한 posterior canal BPPV (PC-BPPV)와 horizontal canal BPPV (HC-BPPV)로 진단된 환자들을 대상으로 후향적으로 설문지 및 의무기록을 검토하였다. 모든 환자에서 신경학적 검사를 통해 중추 신경계의 질환 여부를 확인하였고, 여러 반고리관에 이환된 환자, BPPV에 영향을 줄 수 있는 다른 귀 질환(만성 중이염, 전정신경염, 돌발성 난청, 메니에르병)을 병변측에 함께 갖고 있는 환자, 그리고 난치성 BPPV 환자들은 연구 대상에서 제외하였다.

BPPV의 진단은 2008년 미국 이비인후과학회에서 제시한 진료 지침에 따라 체위성 검사를 통해 각 이환된 반고리관을 결정하였다[13]. 비디오안진검사에서 Dix-Hallpike 검사와 수평면에서의 머리회전검사(supine head roll test)를 통해 BPPV를 진단하였다. 안진의 확인은 video Frenzel goggle system (SLMED, Seoul, Korea)을 사용하였다. 이환된 반고리관의 종류와 방향은 다음 검사에 의해 유발된 안진에 따라 분류하였다. Dix-Hallpike 검사에서 짧은 잠복기를 가지면서 지속시간은 60초 이내로 강해졌다가 약해지며 사라지는 형태의 특징적인 회전성 상향 안진이 관찰되면 PC-BPPV로 진단하였다. HC-BPPV의 향지성 아형(geotropic HC-BPPV, HC-BPPV[Geo])은 Supine head roll test 시 전형적인 향지성 방향의 방향전환성 두위안진(direction changing positional nystagmus, DCPN)을 확인하고 Supine head roll test시 강한 안진을 나타내는 방향을 병변측으로 진단하였다. HC-BPPV의 원지성 아형(apogeotropic HC-BPPV, HC-BPPV[Apo])은 Supine head roll test시 전형적인 원지성 방향의 방향전환성 두위안진을 확인하고, 약한 안진이 유발되는 방향을 병변 측으로 진단하였다. HC-BPPV의 2가지 아형을 진단할 때, 양측 안진의 세기가 같은 경우에는 bow and lean test를 통해 병변의 방향을 결정하였다[14].

처음 내원할 때, 환자들은 수면시작 시의 선호 자세에 대한 설문지를 작성하였다. 환자들은 수면 시 선호하는 자세에 대하여 (1) 똑바로 눕는 자세, (2) 우측으로 눕는 자세, (3) 좌측으로 눕는 자세, (4) 특정한 선호자세 없음의 4가지 종류로 응답하였다. 1주 뒤 다시 내원한 환자들은 이석정복술 이후 자세 변화에 따른 유발 안진이 없음을 확인하였고 다시 방문하지 못한 경우에는 전화설문을 통하여 어지럼 재발 유무를 확인하였다. 이후 환자들에게 1개월, 6개월 12개월마다 어지럼증 재발 유무에 대한 전화 설문 및 수면 자세에 대한 설문을 반복하여 시행하였다. 수면 시 눕는 자세와 BPPV의 이환 방향에 대한 상관관계는 Pearson χ² test, Fisher’s exact test를 통해 분석하였다. 치료 전과 치료 후에 환자들의 병변측 또는 건측 방향에 대한 수면 자세 변화의 차이를 Pearson χ² test를 통해 분석하였다. 수면 자세에 따른 BPPV의 재발간의 관계는 첫 설문 시 우측 또는 좌측으로 눕는 자세라 대답한 환자들 중 BPPV의 병변측 방향과 같은 방향인 환자들을 1년 뒤 수면 자세에 따라 두 개의 군으로 나누어 두 군 간의 재발률을 분석하였다. 두 군은 (1) 치료 후 병변측으로 수면을 유지한 군과 (2) 병변 측 이외의 수면 자세로 변화한 군으로 나누었으며, 두 군에서 BPPV의 재발률을 Kaplan-Meier method with log rank test를 시행하여 비교하였다. 통계학적 분석은 PASW ver 18.0 (SPSS Inc., Chicago, IL, USA)을 사용하였으며, p값이 0.05 미만일 때 통계적으로 유의하다고 판정하였다.

결 과

본 연구의 대상이 된 BPPV환자 460명 중 남자가 115명(25%), 여자가 345명(75%)이었고, 평균 연령은 55.1±14.8세(범위, 17–89세)였다. 어지럼의 발병부터 BPPV의 진단까지의 기간은 평균 5.4±10.1일이었다. 반고리관의 분포를 살펴보면, PC-BPPV환자는 244명(53%), HC-BPPV (Geo)환자는 100명(21.7%), HC-BPPV (Apo)환자는 116명(25.2%)이었다.

전체 460명 환자 중 추적관찰이 이뤄지지 않은 환자는 3명(0.7%)으로, 재발 여부 및 수면자세의 추적 설문은 457명(99.3%)에서 시행하였다. 전체 460명 환자들의 수면 자세 분포는 BPPV 각 아형별로 Table 1과 같다. BPPV의 병변 측과 수면 자세와의 상관관계는 PC-BPPV, HC-BPPV (Geo), HC-BPPV (Apo) 세 가지 아형 모두에서 통계적으로 유의한 상관관계를 보였다(p＜0.001, p=0.001, p=0.005) (Table 2). BPPV 치료 후의 환자들의 수면 자세는 병변측은 감소(187명[40.7%]→74명[16.2%]), 건측은 증가(80명[17.4%]→211명[46.2%]) 하였다(p＜0.001). 똑바로 눕는 자세는 증가(110명[23.9%]→112명[24.5%]), 특정한 선호자세 없음은 감소(83명[18%]→60명[13.1%])하였다(Figure 1).

환자들의 수면 자세 변화 여부에 따라 분류한 두 군 간 최대 1년까지의 재발률은 유의한 차이를 보이지 않았다(p=0.661) (Figure 2).

고 찰

본 연구의 전체 환자들 460명 중 PC-BPPV환자는 244명(53%), HC-BPPV (Geo)환자는 100명(21.7%), HC-BPPV (Apo) 환자는 116명(25.2%)명으로 기존의 다른 연구들에서 보고된 HC-BPPV의 비율 5%–33%보다[15,16] 높은 47%를 보였다. 이는 본 연구의 환자들의 어지럼 증상 발생 후 진단까지의 기간이 평균 5.4±10.1일로, 증상 발현 기간이 짧은 환자들에서 HC-BPPV의 환자 비율이 높아진다는 기존의 연구를 통해 설명될 수 있다[17].

또 본 연구의 전체 환자들의 수면자세 분포는 PC-BPPV와 HC-BPPV (Geo)에서는 우측으로 눕는 자세보다 좌측으로 눕는 자세가 더 많은 수를 보였는데 이는 사람들이 연령의 증가에 따라 심혈관계 상태로 인해 우측으로 눕는 자세를 선호한다는 De koninck 등[18]의 연구 결과와는 다르다[19,20]. 이 차이는 본 연구에 포함된 환자들의 평균 연령이 55.1±14.8세(범위, 17–89세)로, 65세에서 80세 사이의 연령대 환자를 연구 대상으로 한 De koninck 등의 연구보다 젊은 사람들이 훨씬 더 많이 포함된 결과로 보인다.

PC-BPPV와 HC-BPPV 환자들 모두에서 이환 방향과 수면 자세와의 상관관계에 있어 BPPV 병변측으로 눕는 자세가 건측으로 눕는 자세보다 많은 양상을 보였는데 이러한 결과는 기존의 다른 연구들과 비슷한 결과였다[7-9,20]. 이는 난형낭반에서 이탈한 이석이 수면 시 머리의 위치에 따라 중력에 의해 아래쪽에 위치한 귀 방향의 반고리관에 들어가 BPPV가 유발되는 것으로 추정된다[7,8,20].

BPPV 치료 후, 환자들은 병변측으로 눕는 비율이 감소하고 건측으로 눕는 비율은 증가하였다. BPPV발생 후 환자들의 수면 자세에서 병변측으로 머리를 향하는 자세를 피하는 경향이 생김을 알 수 있다. 이는 BPPV 치료 이후 환자들의 삶의 질을 다룬 최근 연구에서 BPPV를 겪은 환자들 중에 심리적인 장애로 인해 활동의 제한이 생기는 경우가 있음을 보고하였는데[21], 본 연구에서도 병변측으로 누웠을 때 어지럼이 생긴 것을 경험한 일부 환자들의 어지럼 재발에 대한 의식적 또는 무의식적인 심리적 두려움으로 인한 결과라 생각된다.

그러나 이석정복술 후 자세 제한의 필요성에 대해서는 논란이 많다[10-12,22]. 본 연구에서 치료 후 병변측으로 수면을 유지한 군과 병변측 이외의 수면 자세로 변화한 군에서 1년까지의 BPPV의 재발률은 유의한 차이가 없었고, 이는 BPPV의 치료율과 재발률에 수면 자세 제한이 큰 영향을 미치지 못한다는 다른 연구들과 비슷한 결과를 나타낸다[10,11]. 이에 반해, Cakir 등[12]은 수면 자세 제한 유무에 따라 BPPV의 치료에 필요한 이석정복술의 횟수가 의미 있는 차이를 보였다고 발표했으며, 몇몇 다른 연구들에서도 수면 자세 제한이 치료에 효과가 있다는 결과를 보여주었지만 연구 표본 대상 환자수가 적은 제한점이 있었다[23-26].

BPPV는 치료 후에도 재발이 5%–50%에 이를 정도로 흔한 질환으로[20,27], 중력에 의해 난형낭으로부터 아래쪽으로 떨어진 이석의 축적에 의해 BPPV가 발생할 수 있다는 가설은 BPPV의 치료와 재발 예방에 있어 수면 자세에 대해 앞으로도 계속 충분히 연구해 볼 이유가 된다. 수면 시 머리 자세 변화에 대한 적절한 모니터링 장치의 사용이 연구의 질을 높일 것으로 생각되고, 또한 머리 자세 변화에 따른 반고리관의 정확한 해부학적 위치 변화와 이석의 이동에 대한 연구도 더 진행되어야 할 것이다. 아울러 수면 자세에 따른 BPPV재발 예방의 단기간 및 장기간 효과 비교 및 수면 자세 제한에 따른 환자의 수면의 질 변화 등은 앞으로 더 수행될 필요가 있을 것으로 보인다.

결 론

PC-BPPV와 HC-BPPV 환자들에서 수면 시 자세와 BPPV의 병변측 사이에는 유의한 관계가 있었다. 그리고 치료 후 환자들의 수면 자세는 의미 있게 병변측 방향을 피하고 건측을 선호하는 양상을 보였다. 그러나 수면 자세의 변화가 PC-BPPV와 HC-BPPV환자들의 1년간의 재발률에 영향을 미치지는 않았다. 이 연구 결과는 BPPV 환자들의 수면 자세에 대한 앞으로의 연구에 도움이 될 것으로 생각된다.
