  • Objectives
    Ménière disease is a clinical syndrome characterized by the four major symptoms of episodic vertigo, sensorineural hearing loss, tinnitus, and aural fullness. Sensorineural hearing loss, especially low frequency, is the characteristic type of audiogram in Ménière's disease. However, it is difficult to distinguish idiopathic sudden sensorineural hearing loss (ISSNHL) with vertigo from the first attack of Ménière disease. The purpose of this study was to investigate the incidence of progression into Ménière Disease from low frequency ISSNHL.
  • Methods
    Two hundred eighty-three patients were included in this study. We classified the patients with ISSNHL according to the hearing loss in audiogram and analyzed how many of them actually progressed to Ménière disease based on diagnosis criteria.
  • Results
    Among the 240 patients, 37.1% (89 patients) were confirmed low frequency ISSNHL and 14.6% (13 patients) of them were diagnosed with Meniere disease.
  • Conclusions
    This study showed that the progression from low frequency ISSNHL to Ménière disease was higher than other frequency ISSNHL, as in other studies.
1861년 프랑스 내과의사인 Prosper Ménière에 의해 일시적으로 발생하는 어지럼, 이명, 난청이 내이의 병변일 수 있음이 처음 보고된 이후로 수많은 연구들이 진행되어 왔다[1]. 메니에르병은 어지럼, 감각신경성난청, 이명 그리고 이 충만감과 같은 4가지 대표적인 증상을 보이는 임상증후군으로 알려져 있으며 원인은 알 수 없는 경우가 대부분이나 해부학적 이상이나 바이러스 감염, 알레르기나 자가면역 질환 등이 추정되고 있다[2]. 메니에르병에서 나타나는 감각신경성난청의 경우 주로 저음역대의 난청이 흔함이 알려져 있다[3]. 이런 이유로 환자가 저음역대 감각신경성난청이 처음 발생하였을 때 이것을 돌발성난청(sudden sensorineural hearing loss)과 구별하기는 쉽지 않다. 세계보건기구와 유럽연합(World Health Organization and the European Union)에 따르면 돌발성난청의 유병률은 100,000당 50명 이하일 정도로 매우 드문 질환이며 메니에르병 또한 유병률이 100,000명당 190명 정도로 보고되어 있다[4-6]. 실제로 저음역 난청 환자들 중 10% 내외에서 메니에르병으로 진행한다는 보고들이 있으나 관련된 연구가 많지 않으며 그 결과가 상이할 뿐만 아니라 최신 메니에르병의 진단 기준을 적용한 보고가 없다. 이에 저자들은 최신 메니에르병의 진단 기준을 적용하여 돌발성난청에서 메니에르병으로 진행되는 빈도 및 난청 음역에 따른 비교, 그리고 그에 따른 청력 호전 정도를 확인하고자 연구를 진행하였다.
이 연구는 2013년도부터 2015년도까지 돌발성난청으로 내원한 환자 283명을 대상으로 하였다. 대상군 중 고막 천공이나 외상, 소음 노출 등 특이 과거력이 있는 환자들은 제외하였으며 악성 종양이나 자가면역질환 등 다른 전신적 질환이 있는 환자 또한 연구에서 제외하였다. 돌발성난청의 진단은 증상이 발생한지 72시간 이내 순음청력검사상 최소한 3개 이상의 연속된 주파수에서 30 dB 이상 역치가 감소한 경우로 정의하였다[7]. 이에 총 240명이 선별되었으며 남성이 137명, 여성이 103명, 평균 나이는 50.5세이며 13세에서 80세까지 분포되었다. 대상군의 청력 역치는 순음청력검사를 통해 확인하였으며 4분법으로 계산하여 평균 50 dB, 표준편차 27.7 dB로 나타났고 범위는 6.3–100 dB이었다. 100 dB 이상의 고도 난청과 무반응 결과는 100 dB로 계산하였다(Table 1). 평균 추적 관찰 기간은 195주이며 시행한 순음청력검사상 청력 저하의 양상에 따라 저음역(0.25–1 kHz), 중간음역(1–4 kHz), 고음역(4–8 kHz) 및 전체음역 난청으로 분류하였다(Fig. 1). 모든 환자들은 고용량 스테로이드(1 mg/kg, 최대 60 mg, 10 days) 치료를 시행하였고, 그 중 저음역 감각신경성난청 환자들은 스테로이드와 동시에 이뇨제(hydrochlorothiazide 25 mg, twice a day)도 함께 복용하도록 하였다. 이뇨제는 모든 저음역 감각신경성난청 환자들에게 2주를 기준으로 처방하였으며 그 전에 난청이 호전이 되면 중단하도록 하였다. 메니에르병으로 진행된 경우는 3개월을 기본으로 복용하였으며 이후 증상 발현 여부에 따라 추가 또는 중단하였다. 회복 여부는 외래 환자인 경우 1주일 뒤에 첫번째 재평가를 시행하였으며 이후 환자 사정에 따라 1주 혹은 2주 간격으로 추적 관찰하였고 입원 환자의 경우 주 3회 순음청력검사를 시행하였다. 메니에르병은 미국 이비인후-두경부 외과학회에서 2015년에 정의한 명확한 메니에르병의 기준에 준하여 진단하였으며(The 2015 report of the Committee on Hearing and Equilibrium of the American Academy of Otolaryngology-Head and Neck Surgery, “definite” Ménière's disease) 청력의 회복은 ‘Siegel’s Criteria‘에 따라 순음청력검사 결과에서 15 dB 이상 호전되었을 경우로 정의하였다(Tables 2, 3) [2,8,9]. 이를 통해 각 음역대별 난청에 따라 메니에르병으로의 진행 및 청력 회복에 대해 분석하였으며 성별에 따른 분석도 함께 시행하였다.
범주형 변수의 경우 빈도수와 비율로 결과를 나타내었고, 연속형 변수는 평균값과 표준편차를 이용해 표현하였다. 난청 음역에 따른 메니에르병으로의 진행률과의 상관관계를 확인하기 위해 각 음역대별로 나누어 Fisher exact test를 사용하여 통계학적 유의성을 확인하였으며, 성별에 따른 진행률 및 메니에르병의 진행 여부에 따른 청력 호전 정도를 분석을 보기 위해 chi-square test를 사용하였다. 모든 통계학적 검증은 IBM SPSS ver. 18.0 (IBM Co., Armonk, NY, USA)를 사용하였으며, p 값이 0.05 미만일 때 통계적으로 유의미하다고 판정하였다.
총 240명의 환자들 중에서 저음역 감각신경성난청 환자는 89명(37.1%), 중간음역 감각신경성난청 환자는 5명(2.1%), 고음역 감각신경성난청 환자는 43명(17.9%), 전음역 감각신경성난청 환자는 103명(42.9%)으로 확인되었다. 증상 발생 후 첫 내원까지 걸린 기간은 평균 8.4일, 표준편차는 10.2일이었으며 240명 중 47명이 타병원에서 약물치료를 먼저 시작하였다. 전체 환자들 중 14명(5.8%)의 환자들이 고용량 스테로이드 복용 이후 경과 관찰 중에 “definite” Ménière disease로 진단되었으며 저음역 감각신경성난청 환자군에서는 13명(14.6%), 전음역 감각신경성난청 환자군에서는 1명(0.97%)으로 확인되었고 중음역 및 고음역 감각신경성난청 환자군에서는 “definite” Ménière disease로 진단된 환자들은 없었다(Tables 4, 5). 청력의 회복 여부를 보았을 때 메니에르병으로 진단된 환자들에서는 14명 중 10명(70.3%)이 회복되었으며 메니에르병으로 진단되지 않은 환자들에서는 226명 중 98명(43.4%)이 회복되었으며 이는 p 값이 0.04로 통계적으로 유의미한 값을 가졌다(Fig. 2). 청력이 회복된 환자군에서 순음청력검사상 난청 발생 시 평균은 50 dB, 표준편차는 27.6 dB이었며 회복 후 평균 28.3 dB, 표준편차는 18.6 dB로 확인되었고, 치료 시작 후 청력 회복까지 걸린 기간은 평균 12.4일, 표준편차 11.3일로 나타났다. 성별에 따른 차이로는 남성 환자들 중에서 메니에르병으로 진행한 환자는 총 131명 중 6명(4.4%) 이었으며 여성 환자들에서는 총 95명 중 8명(7.8%)이었다(Fig. 3). 성별에 따른 메니에르병으로의 진행률을 비교한 통계분석에서 위험도(odd ratio)는 여성대비 남성에서 0.56로 상대적으로 낮게 측정되었으나, p 값은 0.28로 통계적으로 유의미한 차이를 보이지는 않았다.
돌발성난청의 유병률은 100,000명당 3–160명으로 다양하게 보고가 되고 있지만 실제 유병률은 훨씬 더 높을 것이라는 견해가 많다[10]. 이는 자연적인 회복률이 높아 증상이 발생하여도 실제로 병원을 찾지 않는 환자들이 많을 것이라고 생각되기 때문이며 자연 회복률은 45%–65% 정도로 보고 되고 있다[11]. 메니에르병 초기에 발생하는 증상들의 비율을 보면 난청은 22%, 어지럼이 37%, 두 가지 모두 나타나는 경우가 41% 정도로 보고되고 있어 단순히 난청만 있을 경우에는 돌발성난청과 감별이 어렵다고 알려져 있다[12]. 이 연구에서는 메니에르병의 주증상인 난청이 단일 증상으로 발생한 경우 돌발성난청으로 진단될 수 있을 것이며, 그 후 경과 관찰 중에 어지럼이나 이명, 이충감과 같은 증상이 동반되면서 메니에르병으로 진행할 수 있을 것이라 생각했고 실제 그 빈도를 확인하였다.
이 연구에서 저음역 감각신경성난청 환자들에서 14.6%에서 메니에르병으로 진행됨을 확인하였으며 다른 음역의 난청 환자들보다 메니에르병으로 진행될 가능성이 높음을 알 수 있었다. 청력 회복의 경우에도 메니에르병으로 진단된 군에서 71.4%, 그렇지 않은 군에서 43.4%로 나타나 메니에르병의 증상 중 하나인 감각신경성난청의 경우 일시적으로 나타나는 경우가 많아 청력의 회복률이 높음을 마찬가지로 함께 설명할 수 있다. 메니에르병의 성별에 따른 유병률 차이는 일반적으로 여성에서 좀 더 높다고 알려져 있다. 그 차이가 크지는 않으나 여성대 남성의 비가 1.3:1–4.3:1로 다양하게 보고되어 있으며 이 연구에서도 남녀 유병률의 차이가 1.8:1로 여성군에서 좀 더 높게 나타났으나 통계적으로 유의미하지는 않았다[6]. 이런 성별에서의 차이는 메니에르병이 성호르몬에 따른 영향도 있을 것이라 보는 견해도 있으며 성별에 따라 사회적 활동이 다름에서 나타나는 차이라 보는 견해도 있다[13]. 메니에르병의 발병 기전은 내림프 수종과 관련이 있을 것이라 생각되며 이로 인한 내이의 막성 미로 내의 내림프 압력의 증가로 인해 감각신경성난청이나 어지럼, 이명 및 이 충만감과 같은 증상이 발생한다고 알려져 있다[14]. 때문에 내림프 압력을 줄이는 방법으로 저염식이를 통한 염분 섭취의 감소나 이뇨제 복용이 대표적으로 알려져 있으며 여러 연구를 통해 실제 증상 완화 및 악화 방지에 효과가 있음이 입증되었다[15].
이 연구는 후향적 분석을 통해 저음역 감각 신경성 난청이 다른 음역대의 감각신경성난청과 비교하여 메니에르병으로 진행할 가능성이 높다는 사실을 확인하였다. 그러나 환자가 처음 내원하여 저음역 감각신경성난청이 확인되었을 경우 메니에르병과 돌발성난청을 감별하는 것은 여전히 어렵다. 실제 두 질환을 감별하기 위해 전정유발근전위검사를 시행해 보았을 때 유의한 차이가 있어 감별진단에 이를 이용할 수 있다는 보고도 있으나 임상에서 난청으로 내원한 모든 환자에게 검사를 시행하기에는 어려움이 있다[11].
메니에르병 환자가 실제로 돌발성난청으로 잘못 진단되었을 가능성을 고려해 볼 때, 청력의 개선과 관계없이 지속적인 경과 관찰이 필요하다. 특히나 저음역 난청 환자들에게는 메니에르병으로의 진행 가능성 및 치료와 경과에 대해 추가적으로 설명하도록 하고 특히나 어지럼이나 다른 증상이 동반될 시 재내원이 필요함을 설명하는 것이 바람직하다.
