  • Objectives
    The nystagmus evoked by an angular velocity step is influenced by the cupula dynamics and the status of velocity storage mechanisms. This study questioned whether the cupulolithiasis of horizontal canal benign paroxysmal positional vertigo (HC-BPPV) affects the time constant or gain of the velocity step test.
  • Methods
    We performed a retrospective study of 30 patients who diagnosed with HC-BPPV and performed rotary chair test at Dankook University Hospital from June 2010 to May 2017. All patients had normal vestibular function on the sinusoidal harmonic acceleration test. According to the direction of nystagmus on the head roll test, we further divided the patients into “geotrophic” (canalolithiasis) and “apogeotrophic” (cupulolithiasis) groups. We compared the time constant (Tc), gain, and directional preponderance (DP) between the 2 groups. We also compared the time constant and gain between lesion side of BPPV and normal side in each group.
  • Results
    In the supine head roll, geotrophic nystagmus observed in 14 patients and apogeotrophic nystagmus observed in 16 patients. When parameters of the velocity step test compared according to lesion side of BPPV (per-rotary of lesion side, postrotary of lesion side, per-rotary of normal side, and postrotary of normal side), there were no significant differences in Tc, gain, and DP between the 2 groups. There were also no significant differences in Tc and gain between lesion side and normal side in each group.
  • Conclusions
    We assumed that cupulolithiasis of HC-BPPV could affect the cupular deflection evoked by an angular velocity step, but there were no significant differences in Tc and gain between patients with cupulolithiasis and canalolithiasis.

회전의자검사는 자극의 종류에 따라 정현파회전검사(sinusoidal harmonic acceleration)와 등속회전검사(velocity step)로 구분된다. 정현파회전검사는 여러 주파수의 회전자극(0.01–1 Hz)을 주어 나타나는 안진을 각 주파수별로 이득(gain), 위상(phase), 비대칭(asymmetry)을 측정하는 검사이다. 이에 비해 등속회전검사는 회전의자를 정지한 상태에서 시계방향 또는 반시계방향으로 가속하여(100°/sec2) 일정한 속도(100°/sec)로 회전시키다 감속, 정지시킨 후 나타나는 회전 중 안진(per-rotatory nystagmus)과 회전 후 안진(postrotatory nystagmus)을 측정하는 검사이다.
등속회전자극에 대한 안구의 움직임은 전정기능과 속도 저장 기전(velocity storage mechanism)에 의해 영향을 받는데[1,2], 자극 초기에는 수평반고리관(horizonal canal, HC) 내 팽대부릉정(cupula)의 기계적인 반응에 의해 머리회전의 반대방향으로 향하는 안구의 움직임을 보이게 된다. 회전가속이 주어지는 약 1초가 지나면 팽대부릉정이 다시 시간에 대해 지수적으로 본래의 위치로 돌아오게 되고 이에 따라 구심성 전정신경의 반응도 감소하게 된다. 따라서, 등속회전 시에는 안구의 느린성분은 회전반대방향으로 향하며 시간이 지남에 따라 안진의 강도는 점차 감소해 소실되고, 등속회전이 감속으로 정지한 상태에서는 안구의 느린 성분은 회전방향으로 향하며 시간이 지남에 따라 안진의 감도는 점차 감소해 소실된다. 팽대부릉정의 시간상수는 약 5초로 알려져 있으나[3-5], 실제로 나타나는 안구운동의 감퇴는 속도 저장 기전의 양성 되먹임(positive feedback) [6]과 보상(adaptation) 작용[7]에 의해 안진의 시간상수는 10–20초 정도로 연장된다[8].
기존에 보고된 등속회전검사에 대한 연구들은 검사의 통해 측정된 다양한 변수들의 임상적 유용성 및 전정기능평가에 초점을 맞추고 있으나[9,10], 등속회전검사에서 팽대부릉정의 기계적 자극에 영향을 미칠 수 있는 양성돌발성두위현훈(benign paroxysmal positional vertigo, BPPV)이 검사 결과에 미치는 영향에 대해서는 거의 알려진 바가 없다. 수평반고리관의 양성돌발성두위현훈(HC-BPPV)은 이석(otolith)의 위치에 따라 반고리관결석(canalolithiasis)과 팽대부릉정결석(cupulolithiasis)으로 구분할 수 있으며[11,12], 팽대부릉정결석의 경우 팽대부릉정에 이석이 부착되어 굴절(deflection)의 시간이 더 길어질 것으로 생각된다[12]. 따라서, 등속회전검사 시 전정기능이 저하되어 있지 않더라도 HC-BPPV에서 반고리관결석의 경우보다 팽대부릉정결석에서 팽대부릉정의 굴절이 더 오래 유지되어 검사 결과에 영향을 줄 것으로 가정해볼 수 있다. 이에 저자들은 이 연구를 통해 HC-BPPV의 종류(반고리관결석과 팽대부릉정결석)에 따라 등속회전검사에서 팽대부릉정의 기계적인 반응을 반영하는 초기이득, 초기속도 값에 어떠한 영향을 미치는지 알아보고자 하였다.
2010년 6월부터 2017년 3월까지 현훈을 주소로 내원한 환자 중 비디오안진검사와 회전의자검사를 같은 날 시행하고 HC-BPPV를 진단받은 환자들의 의무기록을 후향적으로 분석하였다. 비디오안진검사상 앙와위에서의 머리회전검사(head roll test) 시행 시 두위 변화에 따른 회전성어지럼과 함께 3°/sec 이상의 방향전환성 수평 안진이 관찰되는 HC-BPPV 환자를 대상으로 하였으며, 정현파 회전검사(sinusoidal harmonic acceleration)상 낮은 이득과 비대칭이 관찰되는 경우 전정기능저하가 동반된 것으로 간주하여 대상에서 제외하였다. 또한, 돌발성난청이 동반된 HC-BPPV 환자는 제외하였다.
HC-BPPV 환자들은 안진의 방향에 따라 향지성(geotrophic)안진을 보이는 군과 원지성(apogeotrophic)안진을 보이는 군으로 구분하였다. Ewald의 제2 법칙에 따라 향지성안진을 보이는 군에서는 안진이 더 세게 나타나는 방향을 병변쪽으로 진단하였고, 원지성안진을 보이는 군에서는 안진이 약하게 유발되는 방향을 병변쪽으로 진단하였다.
등속회전검사에 사용된 장비는 Micromedial Technology사(Eden Prairie, MN, USA)의 System 2000으로 정지된 상태에서 일정한 속도에 이르기까지 100°/sec2로 가속하여 100°/sec의 일정한 속도로 유지되는 (1) 회전 중 자극(per-rotary)과 다시 100°/sec2로 감속되어 정지된 상태로 유지되는 (2) 회전 후 자극(postrotary)에 의한 안구운동의 이득과 시간상수를 측정하였다. 분석을 위해 HC-BPPV의 방향에 따라 병변측의 회전 중 자극(per-rotary of lesion side), 병변 측의 회전 후 자극(postrotary of lesion side), 정상 측의 회전 중 자극(per-rotary of normal side), 정상 측의 회전 후 자극(postrotary of normal side)에서 이득과 시간상수를 비교하였다. 방향우위성(directional preponderance, DP)은 100×(병변측‒정상측)⁄(병변측+정상측) (%)의 공식에 의해 구하였는데, 병변 측의 회전 중 자극과 정상 측의 회전 후 자극은 병변측의 전정기능을, 정상 측의 회전 중 자극과 병변 측의 회전 후 자극은 정상 측의 전정기능을 나타내는 것으로 분석하였다.
통계적 유의성을 분석하기 위해 IBM SPSS Statistics ver. 20.0 (IBM Co., Armonk, NY, USA)을 이용하였으며, 안진의 방향에 따라 향지성군과 원지성군으로 구분하였다. 두 군간의 임상 양상, 등속회전검사의 결과를 비교하기 위해 independent t-test, Mann-Whitney U-test, chi-square test를 사용하였다. 또한, 각 군에서 회전 방향에 따른 이득과 시간상수를 비교하기 위해 paired t-test와 Wilcoxon signed rank test를 사용하였다. 통계적 유의성은 p값이 0.05보다 작을 경우에 의미 있는 것으로 판단하였다.
HC-BPPV로 진단받은 30명의 환자들을 분석하였고, 평균 나이는 53±12세이며 성별 분포는 남자 10명, 여자 20명이였다. 이 중 향지성안진을 보인 군은 14명이며, 원지성안진을 보인 군은 16명이었다. Table 1은 향지성군과 원지성군에서 임상 양상과 등속회전 결과를 비교한 결과로, 두 군 간에 나이, 성별, 병변측, 시계 방향 및 시계 반대 방향에서의 회전 중 또는 회전 후 안진의 이득과 시간상수에는 유의한 차이가 관찰되지 않았다.
BPPV가 이환된 병변측에 따라 회전 자극에 대한 이득과 시간상수, 방향우위성을 비교했을 때, 두 군 간의 통계적인 유의미한 차이는 관찰되지 않았다(Table 2). Table 3은 각 군에서 병변측으로 회전했을 때와 정상측으로 회전했을 때의 이득과 시간상수를 비교한 결과이다. 향지성군의 경우 병변측으로 회전 시 정상측으로 회전할 때보다 이득이 낮고 시간상수가 짧은 경향이 있고, 반대로 원지성군의 경우 병변측으로 회전 시 정상측으로 회전할 때보다 이득이 높고 시간상수가 긴 경향이 있었다. 하지만 이러한 경향에는 통계적인 유의미한 차이는 보이지 않았다.
이 연구에서는 팽대부릉정에 이석이 부착되어 있을 것으로 추측되는 팽대부릉정결석에서 등속회전검사 시 팽대부릉정의 굴절이 더 오래 유지되어 이득과 시간상수에 차이가 있을 것으로 가정하였으나, 반고리관결석과 비교했을 때 유의한 차이는 보이지 않았다. 이전 사례에서 HC-BPPV를 진단받은 환자 중 원지성안진을 보이는 경우가 향지성안진을 보이는 경우에 비해 시간상수가 더 길었다고 보고하였으며[13], 수학적 모델에서는 반고리관결석에 의해 유도된 안진이 팽대부릉정결석에 의해 유도된 안진이 비해 완서상 안구운동속도(slow phase eye velocity)가 크다고 보고하였다[14,15]. 하지만 이전 연구의 경우 동일한 환자에서 두위 변환 시 이석이 중력의 방향으로 이동함으로써 발생하는 안진의 방향이 원지성에서 향지성으로 전환될 때 안진의 특성을 비교하였으며[11,16], 등속회전검사에서 HC-BPPV의 아형에 따른 이득 및 시간상수의 비교는 보고된 바가 없다.
이 연구에서 가설과 달리 HC-BPPV의 아형에 따른 이득과 시간상수의 결과에 차이를 보이지 않았으며, 이는 머리회전검사와 등속회전검사 시 팽대부릉정의 굴절에 영향을 미치는 회전축의 방향 및 자극의 종류가 다르기 때문으로 생각한다. 이전 연구에서는 앙와위(supine position)에서 두위 변환 시 이석이 중력의 방향으로 이동함으로써 팽대부릉정의 굴절을 유발하여 발생하는 안진에 대해 비교하였으나, 이 연구에서 비교한 등속회전검사의 경우 지속적인 회전자극 시 전정안반사에 의해서 나타나는 안진을 측정하며, 시간상수에는 등속도 회전자극에 대한 반응시간을 연장시키는 속도저장기전이 관여하게 된다. 따라서, 등속회전검사에서 이석에 의한 팽대부릉정의 기계적 반응을 단독으로 평가하기는 어려울 것으로 생각한다. 또한, 팽대부릉정결석에서 팽대부릉정의 굴절은 중력의 방향에 영향을 받는다고 보고되는데[17], 머리회전검사 시 수평 반고리관의 면은 중력방향과 일치하나 등속회전검사 시 수평 반고리관의 면은 중력방향과 수직을 이루므로 팽대부릉정의 굴절이 중력방향으로 인해 팽대부릉정결석에 큰 영향을 받지 않을 수 있을 것으로 생각한다.
이 연구는 후향적 의무기록을 검토하였기 때문에 등속회전검사에서 초기속도나 안진의 시간, Slow cumulative eye position와 같은 다양한 변수를 비교하지 못한 제약점이 있다. 또한, 이 연구에서 분석 시 안진의 방향이 원지성인 경우 팽대부릉정결석으로 가정하였기 때문에 단완(short arm)에 발생한 반고리관결석이 원지성안진을 보이는 군에 포함되었을 가능성이 있으며, 팽대부릉정의 굴절에 영향을 줄 수 있는 light cupula에 대한 고려가 없었다. 따라서 향후 연구에서는 안진의 지속시간에 따라 지속성 및 일과 성향지성방향전환성두위안진에 대한 하위그룹 분석과 등속회전검사에서 이득, 시간상수 이외에 다양한 변수들에 대한 비교가 필요할 것으로 생각한다.
결론적으로 HC-BPPV의 종류에 따라 등속회전검사의 이득 및 시간상수에는 유의미한 차이는 관찰되지 않았으며, BPPV 환자에서 등속회전검사의 이득과 시간상수의 값이 비정상적으로 나올 시 여러가지 원인이 있을 수 있으므로 조심스러운 해석이 필요할 것이다.
