Abstract

Objectives:
This study aims to investigate the usefulness of the video head impulse test (vHIT) as a method for evaluating vestibular function in patients with unilateral chronic middle ear disease.
Methods:
Nineteen patients with various unilateral middle ear diseases including chronic otitis media (COM), COM with cholesteatoma, and adhesive otitis media successfully underwent preoperative vHIT. The gain of vestibuloocular reflex (VOR) and the presence of corrective saccades were compared between the affected ear and the healthy ear.
Results:
A total of eight male and 11 female patients with a mean age of 43.1 years were included in this retrospective study. Twelve ears had COM, four had COM with cholesteatoma, and three had adhesive otitis media. A positive history of vertigo or dizziness was reported in 36.8% of the cases. The VOR gain of the affected ears and the healthy ears were 0.97±0.16 vs. 1.00±0.08 in the horizontal canal, 0.91±0.11 vs. 0.87±0.11 in the anterior canal, and 0.87±0.17 vs. 0.99±0.12 in the posterior canal, respectively. Only VOR gain of the posterior canal was significantly decreased compared with healthy ears.
Conclusions:
The average VOR gains in the patients with chronic middle ear disease were within the pre-defined values of normality; however, when compared quantitatively, posterior canal gain of affected ears was significantly decreased compared to healthy ears. Our results can be explained by the anatomical proximity of the ampulla of the posterior canal and middle ear space.

서 론

중이와 내이는 해부학적으로 밀접하며, 정원창 혹은 난원창을 통해 중이강에 영향을 미치는 다양한 질환의 염증이 내이로 파급될 수 있고, 이로 인해 감각신경성 난청, 이명뿐만 아니라 어지럼 등의 전정증상을 유발할 수 있다[1]. 최근 여러 연구에서 중이염으로 인한 전정 기능 장애의 가능성을 보여주고 있다. 만성 중이염 환자의 40% 이상이 질병의 경과 동안 어지럼을 경험하는 것으로 보고되었으며[2-4], 여러 연구에서 전정 기능에 대한 만성 중이염의 잠재적인 영향에 의해 회전의자검사, 온도안진검사, 전정유발근전위 검사에서 비정상적인 결과가 나타날 수 있음이 확인되었다[2,3,5]. 그러나 만성 중이질환과 어지럼 사이의 잠재적인 연관성에 대한 연구는 부족한 실정이며, 이는 중이에 병변이 있는 환자에서 전정계의 기능을 안정적으로 평가할 수 있는 방법의 부재에 기인한다. 또한 비교적 최근에 시행된 체계적 문헌고찰(systematic review)에서 이러한 연구의 질적인 문제와 몇 가지 잠재적인 편향(selection, attribution, performance and detection)으로 인해 결과 해석에 주의를 요한다고 보고하였다[4].

전통적으로 온도안진검사를 이용하여 다양한 중이질환을 가진 환자의 전정기능을 평가하려는 시도가 있었으나, 이론적으로 중이에 병변이 있는 상태에서 온도 에너지의 전달 정도를 알 수 없기에 한계가 있었다[6]. 최근에는 중이의 병변을 우회하여 전정계의 기능을 평가할 수 있는 비디오 두부충동검사가 개발되어 중이에 만성적인 염증을 가지고 있는 환자들의 전정기능을 더 정확하게 평가할 수 있게 되었다. 비디오 두부충동검사는 환자의 머리를 빠르게 회전시키면서 머리의 회전속도와 전정안구 반사에 의해 유발된 안구 움직임의 변화값이 각각 측정되며 이를 통해 전정안구반사의 이득을 계산하고 교정성 단속운동을 검사하는 방법으로, 세 쌍의 반고리관의 기능을 독립적으로 측정할 수 있는 유용한 평가 도구이다[7]. 또한, 온도안진검사와 달리 중이 상태를 우회하여 직접적인 전정안구반사의 경로를 평가할 수 있으므로, 중이질환을 가지고 있는 환자에서도 안정적으로 반고리관의 기능을 평가할 수 있다[8].

본 연구에서는 다양한 중이의 병인으로 수술적 처치를 받은 환자들의 수술 전 비디오 두부충동검사를 분석하여 중이질환을 가진 환자의 전정기능을 평가하고자 하였다.

대상 및 방법

1. 대상

2018년 3월부터 2020년 3월까지 조선대학교병원 이비인후과에서 일측 만성 중이염 및 진주종성 중이염, 유착성 중이염으로 수술 치료를 받은 환자의 의무기록을 후향적으로 분석하였다. 모든 환자들은 수술 전 병력 청취, 고막 검진, 청력검사, 비디오 두부충동검사를 시행하였다. 메니에르병, 전정신경염, 이독성 약물 섭취, 내이 기형 등 명확한 다른 내이 및 전정질환의 기왕력이 확인된 환자와 환기관 삽입술을 제외하고 이전에 중이염 수술의 병력이 있는 환자 및 양측 중이염의 기왕력이 있는 환자는 분석에서 제외하였다. 또한 진주종으로 인해 내이 구조 일부가 파괴된 환자도 제외하였다. 대상자의 평균 나이는 43.11±18.22세였다.

본 연구는 헬싱키 선언을 준수했으며 조선대학교병원의 기관윤리심의위원회의 승인을 받았다(No. 2021-08-017). 이 연구는 후향적 연구로 사전 동의(informed consent)를 면제받았다.

2. 비디오 두부충동검사

ICS impulse 3D 비디오 헤드 임펄스 시스템(GN Otometrics, Taastrup, Denmark)을 사용하여 모든 반고리관의 기능을 정량적으로 평가했다. 비디오 두부충동검사는 환자의 임상 양상과 청력검사 결과를 모르는 한 명의 숙련된 검사자가 시행했다. 안구의 위치를 보정한 후 무작위 순서로 오른쪽과 왼쪽의 수평 및 수직 두부충동검사를 150°–250°/초의 최대 머리 속도와 함께 예측할 수 없는 순서로 양쪽으로 최소 20회 반복하였다. 전정안구반사의 이득은 머리 속도 영역에 대한 안구 속도 영역의 면적비로 계산하였다. 전체 두부충동검사 중에 20% 이상 비슷한 진폭과 잠복기를 가지고 교정성 단속운동(overt or covert)이 발현한 경우 병적인 단속운동으로 판단하였다. 검사 중 나타날 수 있는 부적절한 검사결과(artifacts)를 줄이기 위해 최고 각속도를 150°/초 이상으로 유지하였고, 눈 깜빡임, 머리 회전 후 반동, 잘못된 안구각 보정, 부적절한 동공 추정 등은 분석에서 제외하였다[9]. 전정안구반사의 이득이 수평 반고리관에서 0.80 이상, 수직 반고리관에서 0.70 이상인 경우 정상으로 정의하였다[7]. 반고리관의 기능 이상은 전정안구반사의 이득이 정상 이하로 감소하거나 명확한 교정성 단속운동(overt or covert)이 관찰되었을 경우로 정의하였다.

3. 통계 분석

모든 변수들은 평균값±표준편차로 제시하였고 유의수준이 0.05 이하인 경우 통계적으로 유의하다고 판정하였다. 정상 귀와 병변 귀의 청력과 전정안구반사의 이득을 비교하기 위해 데이터가 정규분포를 따를 경우 paired t-test, 따르지 않는 경우 Wilcoxon test를 사용하였다. 모든 통계적 처리는 IBM SPSS Statistics for Windows, ver. 26(IBM Corp., Armonk, NY, USA)를 이용하였다.

결 과

이 예비연구에서는 앞서 기술한 포함 및 배제 기준에 따라 총 19명의 대상자의 의무기록을 분석하여 얻은 자료를 바탕으로 연구를 진행했으며, 이중 8명은 남성이고 11명은 여성이었다(평균 나이, 43.11±18.22세). 19명의 환자 중 12명은 만성 중이염, 4명은 진주종성 중이염, 3명은 유착성 중이염으로 7명은 우측 귀, 12명은 좌측 귀에 수술을 시행하였다. 제1형 고실성형술은 6명, 폐쇄동 고실유양돌기절제술(canal wall up tympanomastoidectomy)은 10명, 개방동 고실유양돌기절제술(canal wall up down tympanomastoidectomy)은 3명에서 시행되었다.

총 7명(36.8%)의 환자가 수술 전 다양한 정도의 어지럼 및 균형장애를 호소하였다. 문진상 명확한 회전성 어지럼을 경험한 환자는 없었으며, 7명 모두 간헐적인 균형장애 및 아찔함을 호소하였다. 환자들의 병리적 특성 및 수술 방법에 대한 세부정보는 Table 1에 기술하였다. 수술 전 평균 기도청력 역치는 3분법으로 평가 시 병변 귀는 35.68±11.98 dB, 정상 귀는 13.58±7.24 dB였다. 환자의 청력에 대한 세부정보는 Table 2에 제시하였다.

각 반고리관의 전정안구반사의 이득, 비정상 이득의 비율, 교정성 단속운동(overt or covert)의 유무는 Table 3에 기술했다. 수술 전 병변 귀에서 비정상적인 전정안구반사의 이득을 보이는 경우는 수평반고리관에서 3예(15.8%), 전반고리관에서 1예(5.3%), 후반고리관에서 2예(10.5%) 관찰되었다. 병변 귀의 평균 전정안구반사의 이득은 수평, 전, 후반고리관에서 각각 0.97±0.16, 0.91±0.11, 0.87±0.17이었으며, 정상 귀의 평균 전정안구반사의 이득은 1.00±0.08, 0.87±0.11, 0.99±0.12로 앞서 정의된 정상 수치 이내였다. 그러나 양측의 전정안구반사 이득을 정량적으로 평가하였을 시 병변 귀의 후반고리관 이득은 정상 귀에 비해 통계적으로 유의하게 감소되어 있음을 확인할 수 있었다(p=0.017, paired t-test) (Fig. 1).

교정성 단속운동은 병변 귀의 수평반고리관에서 4예, 전반고리관과 후반고리관에서 각각 1예씩 관찰되었다. 병변 귀와 정상 귀의 비정상 전정안구반사 및 교정성 단속운동(overt or covert)의 발현율은 통계적으로 차이를 보이지 않았다. 병변 귀에서 한 개 이상의 반고리관에서 비정상 전정안구반사 및 교정성 단속운동(overt or covert)이 발현된 경우는 6명(31.6%)이었으며, 13명의 환자(68.4%)는 모든 반고리관에서 정상적인 전정안구반사의 이득이 확인되었다.

고 찰

중이의 병변이 현기증 및 어지럼을 유발할 수 있다는 것은 잘 알려져 있는 사실이다[4,10,11]. 중이 내의 바이러스나 세균 등의 감염성 물질은 난원창, 정원창, 또는 진주종성 중이염에 속발한 골부누공을 통해 내이로 침범하게 되며 다양한 정도의 난청과 이명 및 어지럼이 나타난다. 따라서, 난형낭, 구형낭 및 반고리관을 포함한 전정기관은 중이의 만성 감염 혹은 염증에 영향을 받을 수 있다[12]. 다양한 임상 연구에서 만성 중이질환이 전정기능검사에서 이상 소견을 유발할 수 있다고 보고하였다[1-3,5,6]. 2008년 Gianoli와 Soileau [6]는 만성 화농성 중이염 환자에서 온도안진검사와 회전의자검사의 상관관계를 보고하면서, 만성 중이염 환자에서 전정 기능을 평가하기 위한 온도안진검사 시행의 타당성을 제시하였다. 그러나 최근의 체계적 문헌고찰에서 이러한 연구들이 포함/배제 기준 및 교란 변수에 대한 통제가 부족하고 몇 가지 잠재적인 편향이 확인되었다고 보고하기도 했다[4].

본 연구에서는 내이에 영향을 미치는 다른 요인들을 통제하기 위해 반대 측에 중이염 및 다른 전정질환의 기왕력이 있는 환자는 배제하였고, 반대 측에 정상 내이기능을 가지고 있는 일측 만성 중이질환만을 대상으로 연구 초점을 맞췄다. 따라서 본 연구의 결과는 중이의 만성 염증 및 감염이 반고리관 기능에 미치는 영향을 보다 실제적으로 평가했다는 의미가 있다.

비디오 두부충동검사 결과, 6명(31.6%)에서 한 개 이상의 반고리관에서 이득 저하 및 교정성 단속운동(overt or covert)이 발현하였음이 확인되었다. 수평반고리관에서 이득이 0.8 이하인 경우는 3명이었고, 교정성 단속운동(overt or covert)이 발현된 경우는 4명이었다. 이 중 2명은 이득 저하 없이 교정성 단속운동(overt or covert)이 확인된 경우였다. 평균 전정안구반사의 이득은 세 쌍의 반고리관에서 모두 정상 범위를 보였다(Fig. 1). 이와 같은 결과는 비디오 두부충동검사가 중추성 보상이 된 경우 반고리관에 미치는 영향을 정확하게 감지하지 못할 수 있고[13], 주로 고주파수의 전정안구반사를 측정하기 때문에 더 낮은 주파수에서 발생하는 반고리관의 기능 이상을 놓칠 수 있기 때문으로 생각된다[7].

본 연구에서 대상자의 약 36.8%가 어지럼 및 현기증의 기왕력을 가지고 있었다. 그러나, 실제 비디오 두부충동 검사에서 이상 소견을 보인 경우는 6명이었으며, 증상 여부와 전정기능검사 결과 사이에 유의미한 결과를 도출하지는 못하였다. 이는 질병의 경과 동안 중추 보상 메커니즘으로 인해 비정상적인 전정기능검사 결과를 보이는 모든 증례에서 자각 증상을 호소하는 것은 아닌 것으로 생각된다. 또한 반고리관의 기능 이상이 주관적인 증상이 없는 환자에게 나타날 수 있으며, 적절한 검사를 시행하지 않으면 간과될 수 있음을 의미한다.

본 연구에서 병변 귀의 후반고리관 전정안구반사 이득이 정상 귀에 비해 통계적으로 유의하게 떨어져 있음이 확인되었다. 이는 내이와 중이의 해부학적 접근성으로 설명할 수 있다. 정원창 및 난원창에서 후반고리관 팽대부까지의 거리가 수평반고리관이나 전반고리관에 비해 가깝게 위치하기 때문이다[14]. 따라서 염증물질이 직립 상태에서는 중력의 영향으로 보다 쉽게 후반고리관 팽대부에 영향을 미칠 수 있을 것으로 예상해 볼 수 있다. 이와 더불어 전정계의 혈관 분포도 한 가지 원인이 될 수 있다. 앞전정동맥(anterior vestibular artery)은 수평 및 전반고리관 팽대부를 공급하며, 상전정신경의 분포와 일치한다. 후반 고리관은 주로 뒤전정동맥(posterior vestibular artery)에 의해 공급받는다[15]. 이러한 혈액 공급의 차이와 혈관의 선택적 취약성이 본 연구의 결과의 일부를 설명할 수 있다.

본 연구는 19명의 일측성 만성 중이질환을 가진 환자의 의무기록을 후향적으로 분석한 예비연구로, 몇 가지 제한점이 있다. 우선 이 연구에 포함된 대상자의 수가 다른 연구에 비해 적었는데, 이는 본 예비연구 이후 후속 연구를 통해 극복할 수 있을 것으로 생각된다. 또한 후향적 연구의 특성상 환자 선정 기준에 따른 편향을 완전히 배제할 수 없었고, 두부충동검사 외 다른 전정기능검사를 시행하지 못해 대상자가 전정질환을 가지고 있을 가능성을 완전히 배제하지 못하였다. 그러나 일측의 만성 중이 질환을 가진 환자의 전정안구반사를 정상 귀와 정량적으로 비교하여 잠재적인 반고리관의 기능 이상을 평가한 연구로서 의의가 있으며, 후속 연구를 통해 더 많은 환자를 대상으로 반고리관의 기능을 평가하여 만성 중이질환에서 내이의 병태생리를 밝히는 노력을 하고자 한다. 또한 비디오 두부충동검사가 만성 중이질환을 가진 환자의 전정기능을 실용적으로 평가할 수 있음을 보여주었다.

결 론

비디오 두부충동검사는 온도안진검사에 비해 비교적 덜 침습적이며, 중이를 우회하여 반고리관의 기능을 평가하기 때문에 전정기능을 안정적으로 평가할 수 있고, 이는 수술 후 변화를 비교할 수 있는 기준선의 역할을 할 수 있을 것으로 생각된다.
