  • Objectives:
    This study aimed to investigate clinical significance of a head-bending test in benign paroxysmal positional vertigo (BPPV) involving the posterior semicircular canal (PC-BPPV).
  • Methods:
    We retrospectively recruited 256 patients with unilateral PC-BPPV between January 2016 and December 2021, and assessed the clinical characteristics of patients showing head-bending nystagmus (HBN).
  • Results:
    Of 256 patients, 138 (53.9%) showed HBN. Most patients (n=136, 98.6%) had downbeat nystagmus with (n=38) or without (n=98) torsional component. The remaining two patients had pure upbeat and torsional nystagmus, respectively. The torsional component was directed to the contralesional side in all. Between patients with and without HBN, there were no significant differences in clinical characteristics such as age, lateralization, types of BPPV (canalolithiasis or cupulolithiasis), and success rate of repositioning maneuver.
  • Conclusions:
    Head-bending test may be useful in predicting the diagnosis and lateralization of PC-BPPV.
양성돌발두위현훈(benign paroxysmal positional vertigo)이 가장 흔하게 발생하는 곳은 뒤반고리관이다[1,2]. 이는 고개 움직임에 따른 어지럼을 호소하면서 Dix-Hallpike 수기에서 발작성 상향-회선안진이 있을 때 진단할 수 있다[3]. 양성돌발두위현훈의 치료에는 이석정복술이 필요하며 이를 위해서는 정확한 진단과 이환된 귀의 확인이 중요하다. 하지만 Dix-Hallpike 수기는 고개를 젖히는 행동과 누웠다가 앉는 자세를 반복해야 하는 검사이기 때문에, 고령이나 척추질환이 있는 환자에게 시행하기 어려운 경우가 많고, 심한 어지럼으로 검사에 협조가 되지 않는 경우도 있다[4].
머리숙임검사(head-bending test)는 앉아서 고개를 90° 숙여 안진을 관찰하는 방법으로, 수평반고리관 양성돌발두위현훈에서 방향을 결정하는 데 도움을 줄 수 있다[5]. 수평반고리관형 양성돌발두위현훈에서 누워머리돌리기 검사(supine head roll test)시 안진의 강도나 주관적 증상이 비슷하여 병변 측을 감별하기 힘든 경우, 머리숙임검사와 바로눕기검사(lying-down test)에서 관찰되는 안진의 방향으로 이환된 귀를 결정할 수 있다. 뒤반고리관 양성돌발두위현훈에서도 머리숙임검사 시 상향 혹은 하향안진이 관찰될 수 있다[6-8]. 뒤반고리관의 해부학적 구조를 고려했을 때, 앉은 자세에서 피치면(pitch plane)을 따라 고개를 움직여도 중력에 의한 이석의 움직임으로 안진이 보일 수 있다. 따라서 뒤반고리관 양성돌발두위현훈에서도 머리숙임검사가 양성돌발두위현훈의 진단이나 이환된 귀를 결정하는 데 도움을 줄 수 있을 것으로 예상되지만, 이에 대한 연구는 드물다. 이에 본 연구에서는 뒤반고리관 양성돌발성현훈 환자에서 머리숙임안진(head-bending nystagmus)의 유무와 방향, 진단적 가치를 평가해 보고자 하였다.
본 연구는 양산부산대학교병원 생명윤리연구심의위원회의 승인을 득한 후 수행하였다(No.05-2022-135). 후향적 연구 특성상 환자동의서는 면제 받았다.
2016년 1월에서 2021년 12월까지 양산부산대학교병원 신경과 어지럼 클리닉에서 뒤반고리관형 양성돌발두위현훈으로 진단된 299명의 환자들을 대상으로 하였다. 이 중 척추 병변이나 고도 비만, 심한 통증으로 모든 체위검사를 시행할 수 없었거나 다중반고리관 양성돌발두위현훈으로 진단된 경우, 이환된 귀를 정확하게 확인할 수 없었던 환자들을 제외한 후 최종적으로 총 256명의 환자를 대상으로 분석하였다.
뒤반고리관 양성돌발두위현훈의 진단기준은 (1) 자세 변화에 따른 반복적인 어지럼을 호소하면서 (2) Dix-Hallpike 수기에서 상향-회선안진이 어지럼과 동반되어 나타나는 것으로 하였다[3]. 또한 상향-회선안진의 지속 시간에 따라 안진이 1분 이내로 소실되는 경우를 반고리관결석증(canalolithiasis)으로, 1분 이상 지속되는 경우를 팽대부마루결석증(cupulolithiasis)으로 분류하였다[3]. 다른 질환의 감별을 위해 모든 환자는 자발안진, 주시유발안진, 두부충동검사, 상하지 실조와 균형기능검사를 포함한 신경이 과적 검사를 받았고, 필요할 경우 뇌자기공명영상을 시행하였다. 치료는 Epley 수기를 시행하였으며, 첫 번째 치료 후 1시간째 재시행한 Dix-Hallpike 수기에서 안진과 증상이 모두 호전된 경우를 치료 성공으로 결정하였다. 치료에 반응이 없는 환자에게는 추가로 Epley 수기 혹은 Semont 수기를 시행하였다.
머리숙임안진은 3차원 비디오안구운동검사(video-oculography)를 이용하여 시고정을 억제한 상태에서 기록하였다. 환자에게 앉은 자세에서 앞을 보게 한 후, 고개를 피치면에서 90° 빠르게 숙였을 때 발생하는 안진을 확인하였다. 안진의 방향과 성분을 분석하였으며 회선 방향은 환자의 입장에서 결정했다.
통계적 검증은 IBM SPSS Statistics ver. 27.0 (IBM Corp., Armonk, NY, USA)을 이용하였다. 모수적 통계로는 Student t-test, 비모수적 통계법으로는 Fisher test를 통해 각 군의 차이를 비교하였고, 유의한 p값은 0.05 미만으로 해석하였다.
총 256명의 뒤반고리관 양성돌발두위현훈 환자 중 138명(53.9%)이 우측 귀에서, 나머지 118명(46.1%)이 좌측 귀에서 발생하였다. 안진의 지속 시간에 따라 분류한 반고리관결석증은 219명(85.5%)이었고, 팽대부마루결석증은 37명(14.5%)이었다.
머리숙임안진은 138명(53.9%)에서 관찰되었으며, 순수 상향안진과 회선안진을 보인 1명씩을 제외한 136명에서 모두 하향안진이 확인되었다. 이 중 38명(27.9%)이 회선안진을 동반하고 있었으며, 우측 후반고리관 병변에서는 모두 반시계 방향, 좌측 후반고리관 병변에서는 모두 시계 방향이었다(Fig. 1). 회선안진만을 보인 환자는 좌측 후반고리관 병변을 가진 환자로 회선 방향은 시계방향이었다. 하지만 머리숙임안진이 있는 군과 없는 군에서 나이와 이환된 귀, 안진 세기의 차이는 없었으며, 반고리관결석증과 팽대부마루결석증에서도 머리숙임안진이 비슷한 비율로 관찰되었다. 다만 머리숙임안진이 없는 군에서 여성의 비율이 더 높았으며, 통계적으로 유의미한 차이를 보였다. 치료 성공률은 머리숙임안진이 있는 군에서 약간 높게 나왔지만, 통계적으로 유의미한 차이는 보이지 않았다. 회선안진 유무에 따른 치료 성공률도 통계적으로 유의미한 차이는 없었다(Table 1).
뒤반고리관 양성돌발두위현훈 환자의 53.9%에서 머리숙임안진이 관찰되었고 대부분 하방안진이었으나 일부에서는 회선안진이 동반되었다. 회선안진은 우측 뒤반고리관 양성돌발두위현훈에서는 반시계 방향으로, 좌측 뒤반고리관 양성돌발두위현훈에서는 시계 방향으로 관찰되어 이환된 귀를 결정하는 데 도움을 줄 수 있었다. 하지만 머리숙임안진의 유무와 다른 임상증상의 연관은 없었으며, 치료 결과에도 영향을 주지는 않았다.
본 연구에서는 머리숙임안진이 관찰된 뒤반고리관 양성돌발두위현훈 환자는 53.9%로, 이는 지금까지 보고된 25%–47%보다 높았다[6,8,9]. Choi 등[7]이 보고한 바에 의하면 뒤반고리관 양성돌발두위현훈 환자 중 머리숙임검사에서 안진을 보인 환자는 47%이나, 머리숙임검사와 머리젖힘검사(head lean test) 중 한 가지에서라도 안진이 확인되는 경우는 전체 113명 중 85명으로 75%였다. 이 중 머리숙임검사 시 하향안진, 머리젖힘검사 시 상향안진을 보이는 양상이 52%로 가장 많았다. Choo 등[8]이 보고한 연구에서는 머리숙임검사 시 하향안진, 머리젖힘검사 시에는 안진이 보이지 않는 패턴이 84%로 대부분이었다. 다른 연구에서도 안진이 있는 경우에는 머리젖힘검사에서는 Dix-Hallpike 수기에서 보이는 안진과 방향이 같았으며, 머리숙임검사에서는 반대 방향으로 확인되었다. 하지만 모든 연구에서 머리숙임안진의 유무와 치료 효과는 연관성이 없었다. 본 연구에서도 같은 결과를 보였으나, 첫 번째 치료에 대한 반응만 고려한 것으로 추가로 이석정복술을 시행한 결과 및 이석정복술 시행 횟수는 고려되지 않았다.
수평반고리관 양성돌발두위현훈에서는 뒤반고리관 양성돌발두위현훈보다 높은 85% 정도에서 머리숙임안진이 나타나는 것으로 알려져 있다[5]. 이는 뒤반고리관의 해부학적인 구조가 피치면과 완전히 일치하지 않고 약 56° 정도 후외방으로 위치하기 때문이라고 예측할 수 있다[10]. 이전 연구에 따르면 머리숙임검사를 피치면이 아닌 우측 앞반고리관–좌측 뒤반고리관(right anterior–left posterior canal)과 좌측 앞반고리관–우측 뒤반고리관(left anterior–right posterior canal)을 따른 면(plane)을 따라 머리숙임검사를 하면 진단율이 더 높아진다는 결과도 있었다[6].
대부분의 안진은 하방안진으로 확인되었고 회선안진이 확인된 환자는 39명(27.9%)이었다. 뒤반고리관 양성돌발두위현훈에서는 Dix-Hallpike 수기 시 내림프액의 팽대부 반대 방향 흐름(ampullofugal flow)으로 인해 흥분성 안진인 상향-회선안진이 관찰되며, 회선안진은 병변 측으로 발생한다. 반대로 머리숙임검사에서는 이석이 팽대부 쪽으로 움직이면서 내림프액의 팽대부 방향 흐름(ampullopetal flow)이 형성되고, 이로 인해 억제성 안진인 하향-회선안진이 관찰되며 회선안진은 병변 반대 측으로 발생할 수 있다(Fig. 2). 즉 머리숙임안진의 회선안진은 우측 뒤반고리관 병변은 반시계 방향, 좌측 뒤반고리관 병변은 시계 방향이라 예측할 수 있다. 본 연구에서도 회선안진이 확인되었던 39명 모두 예상과 일치하였다. 머리숙임검사 시 회선안진이 없이 하방안진만을 보이는 것은 이석이 이동하는 뒤반고리관의 비팽대부릉(nonampullary arm) 쪽이 좀 더 시상면(sagittal plane)에 가까워서 수직안진 성분이 회선안진 성분보다 크기 때문이라 생각된다[11].
지금까지 보고된 뒤반고리관 양성돌발두위현훈에서 머리숙임안진을 확인하는 연구에서는 상향안진은 한 명만 보고되었다[8]. 이 연구에서는 상향안진이 기록에 오류로 보인 것이라고 판단했다. 본 연구에서도 1명의 환자에서 머리숙임검사 시 상향안진이 확인되었다. 이는 검사 상의 오류일 가능성도 있겠으나, 뒤반고리관에서의 이석의 위치 차이로 설명할 수 있다. 이석이 뒤반고리관의 단완(short arm)과 장완(long arm) 사이에 위치해 Dix-Hallpike 수기와 머리숙임검사 모두에서 우연히 팽대부 반대 방향 흐름을 만들었을 가능성이 있다(Fig. 3). 본 연구에서 상향 안진을 보였던 환자는 회선안진은 동반하지 않았으며 치료에 바로 반응이 있었다.
결론적으로 뒤반고리관 양성돌발두위현훈에서 머리숙임안진이 관찰될 수 있으나 모든 뒤반고리관 양성돌발두위현훈에서 보이는 것은 아니다. 하지만 머리숙임검사 시 하향안진의 확인으로 뒤반고리관 양성돌발두위현훈의 진단에 도움을 줄 수 있다. 특히 회선안진의 방향을 확인해 수평반고리관 양성돌발두위현훈과 마찬가지로 이환된 귀를 결정하는 데 도움을 줄 수 있기 때문에 Dix-Hallpike 수기를 시행하기 어려운 환자에서의 진단과 이환된 귀의 확인에 도움을 줄 수 있을 것으로 기대된다.
