Trans Abstract

Ménièreʼs disease is a poorly understood disorder of the inner ear characterized by intermittent episodic vertigo, fluctuating hearing loss, ear fullness and tinnitus. In this paper, we present a review of the histopathology, cytochemistry, gene, blood-labyrinthine barrier and imaging of Ménièreʼs disease. Histopathology is significant for neuroepithelial damage with hair cell loss, basement membrane thickening, perivascular damage and microvascular damage. Cytochemical alterations are significant for altered AQP4 and AQP6 expression in the supporting cell, and altered cochlin and mitochondrial protein expression. Since the discovery of aquaporin water channels (AQP1, AQP2, AQP3, AQP4, AQP5, AQP6, AQP7 and AQP9), it has become clear that these channels play a crucial role in inner ear fluid homeostasis. Several gene studies related to Ménièreʼs disease have been published, but there is no clear evidence that Ménièreʼs disease is associated with a special gene. Currently, imaging techniques to determine the extent and presence of endolymphatic hydrops are being studied, and further studies are needed to correlate the visualization of the endolymphatic hydrops with clinical symptoms.

서 론

메니에르병은 일시적으로 반복되는 발작성 어지럼증, 변동성 난청, 이명, 이충만감의 4대 주 증상을 특징으로 하는 내이 질환으로 보행 장애, 자세 불안정, 낙상 및 오심 등의 증상도 나타날 수도 있다. 메니에르병이라는 명칭은 프랑스 의사인 Prosper Meniere (1799–1862)의 이름에서 유래된 것으로, 1861년 Prosper Meniere가 이러한 병의 원인이 뇌 brain origin에 있는 것이 아니라 내이 전정기관의 장애 labyrinthine origin 라고 최초로 보고하였다. 이후 1938년에 영국과 일본에서 개별적으로 사람의 측두골 연구를 통하여 최초로 메니에르병의 내이 조직병리는 내림프수종 endolymphatic hydrops라고 발표하였다. 측두골내 내림프수종의 원인으로 간주할 만한 뚜렷한 비정상 소견은 없어 “원인불명 idiopathic”이라 기술되고 있다. 내림프수종은 내이에서 내림프공간이 외림프공간으로 확장되는 소견으로 주로 와우와 구형낭에서 흔히 발생하지만, 병이 진행하면서 난형낭과 세반고리관도 포함된다. 메니에르병은 최종적으로 내림프수종 소견을 보이지만 그외 와우와 전정내 신경세포들의 세포 화학적 변화들, 혈-내림프, 혈-외림프를 경계짓는 격막들의 변화들이 나타난다. 고실내 조영제 주사 후 촬영된 자기공명영상(magnetic resonance imaging, MRI)으로 내림프공간과 외림프공간의 시각화가 가능해 져, 내림프수종을 생체 내에서 확인 할 수 있게 되어 진단에 이용되고 있다. 본 종설에서는 최근 보고되고 있는 메니에르병의 병리 조직학적 소견, 생체적 격막, 유전학적 및 MRI에 대해 문헌 고찰 함으로써 연구 경향을 소개하고자 한다.

본 론

1. 사람 측두골 조직병리학적 연구

1938년에 영국의 Hallpike와 Cairns [1] 및 일본의 Yamakawa [2]가 개별적으로 사람의 측두골 연구를 통하여 최초로 메니에르병의 내이 조직병리는 내림프수종 endolymphatic hydrops라고 발표한 이래 많은 연구가 있어 왔다. 메니에르병 초기에 내림프수종은 주로 와우관(cochlear duct)과 구형낭(saccule)에 국한되어 관찰되나, 수종이 진행되면 난형낭(utricle), 팽대부(ampulla) 및 전 내림프 시스템까지 침범하게 된다. 심한 경우 Reissner막이 전정계(scala vestibule) 벽에 부착될 수 있고, 구형낭 벽이 등골 족판을 포함한 반대편 전정벽까지 신전하는 경우도 있다[3]. 내림프수종은 메니에르병과 같이 측두골에서 원인으로 간주할만한 명확한 이상이 없는 경우인 원인미상 idiopathic과 내이에 다른 병적인 변화가 같이 존재하는 이차성 secondary으로 나눠진다. 이차성 내림프 수종을 일으키는 질환은 상염색체 우성유전(nonsydromic deafness autosomal dominant) 9, Alport 증후군, 장액성 미로염, 화농성 미로염, 이성 매독, 측두골 횡골절, 등골 절제술, 인공와우이식술, 내림프낭 종양, 청신경종양, 백혈병, 일차성 면역 매개 내이질환, 결절성 다발동맥염, Cogan 증후군, 궤양성 대장염, 이경화증과 Paget씨병이 있다[4].

내림프수종의 형태학적 연구 외에 Ishiyama 등[5]은 메니에르병 환자의 조직병리학적 연구에서 전정 신경상피(유모 세포) 손상, 기저막 비후, 주위혈관 내피세포 손상, 미세혈관의 심한 손상이 관찰되었고, 흥미롭게도 이석막의 위축 및 퇴행 소견을 관찰했다고 보고하였다. 그리고 세포화학적 연구에서는 AQP4과 AQP6 발현, 미토콘드리아 translocase 발현과 코클린의 변질 소견을 관찰 하였으며, 변이된 aquaporin이 내림프수종을 일으킬수 있다고 설명하고 있다[5].

청력과 균형의 기능을 유지하기 위해서는 내이내 물의 항상성이 필수적이다. Aquaporin 채널이 발견 된 이래로 이들 채널이 내이 유체 항상성에 중요한 역할을 한다는 것이 밝혀졌고, 실제로, AQP1, AQP2, AQP3, AQP4, AQP5, AQP6, AQP7 및 AQP9의 단백질 또는 mRNA가 내이에서 발현되었다[6]. 이들 중 대부분이 내림프의 분비 및 흡수에 관여하는 혈관 선조 stria vascularis와 내림프낭에서 주로 발현되었다[6]. 또, 최근 기전 연구에서 스트레스 호르몬인 바소프레신과 수용체 및 AQP 2 활성화가 내림프수종을 일으키는 기초가 될 수 있다고 보고 하였다[7,8]. 따라서, 메니에르병과 관련된 aquaporin의 연구의 발전은 중요하게 여겨진다.

내림프 배수 방식 endolymph drainage system에 대한 연구가 보고되고 있다[9-11]. 난원낭-내림프(Uticulo-endolymphatic) 밸브는 1928년 Bast에 의해 처음 소개 되었는데, Bast 밸브는 pars superior (세반고리관과 난원낭)에서 pars inferior (구형낭과 와우)와 내림프낭으로 가는 내림프의 유출을 조절하는데, 구체적으로 pars inferior에서의 갑작스런 압력변화를 pars superior에 전달하는 것을 막아주는 기능을 할 것으로 추정한다(Fig. 1). 즉, Bast 밸브가 열리게 되면 메니에르병이 진행이 일어나는데, 내림프낭의 흡수 기전에 손상이 되고 감각상피는 압력변화에 더욱 취약해져 전정 증상을 일어난다[12]. 이러한 증상은 Paparella가 청력소실이 없는 전정 메니에르병으로 보고한 적이 있다[13].

내림프낭은 메니에르병의 병태 생리에 중요한 역할을 하고 있는데, 크기가 작은 내림프관과 낭은 메니에르병에 일으키는 위험 인자와 연관 있다는 보고가 있다[14,15]. Bloch와 Friis [14]은 인간 측두골 검사에서 메니에르병 환자의 내림프낭의 크기가 정상에 비해 의미 있게 감소되어 있다고 보고 하였다.

메니에르병은 Prosper Meniere의 첫 번째 소개 이후 조직병리학적 및 병태생리학적 변화의 연구가 많이 진행되었지만, 임상 증상으로 이어지는 병태 생리는 여전히 논쟁 중에 있다. 향후 사람 조직에서의 분자 연구를 통해 좀 더 밝혀 질 수 있을 것으로 기대한다.

2. 미로막 격막 Labyrinth Membranous Barriers

생체적 격막 barrier은 투과력을 조절해 어떤 물질의 이동을 제한함으로써 격막 전후 공간의 생체 항상성을 유지하는데, 미로막 격막은 특히 내이 림프액의 동적 균형을 유지시키는데 관여한다. 내이 항상성은 내이 림프액의 분비와 흡수, 내이 혈액 공급 이온 수송 및 미로막 격막 체계 labyrinth membranous barriers의 온전성에 의존하는데, 이런 막 미로는 격막 체계에 의해 여러 부분 즉, 내림프, 외림프, 뇌척수액, 혈장으로 나뉘어 진다, 최근에 여러 격막 즉, 혈-내림프 격막, 혈-외림프 격막, 뇌척수액-외림프 격막은 ‘미로막 격막’ labyrinth membranous barrier로도 불리어지며 내이 질환과 미로막 격막 연구가 많이 이루어지고 있다[16-19].

Ishiyama 등[17]은 사람 측두골 연구에서 전정의 난형낭 황반이 메니에르병일때 미로막 격막의 초미세 구조조직이 정상과 다르다고 보고하였는데, 중등도 메니에르병 경우는 혈관 내피 세포내 수많은 소포가 관찰되었고, 아주 심한 경우는 전정기질 vestibular stroma 내의 기저막 비후와 부종성 변화 소견을 보였다.

혈-외림프 격막 blood perilymph barrier는 내이 조직의 혈류와 림프액을 분리하는데 중요한 역할을 담당하고 있다. 급성 염증 시기에는 혈액의 용질과 자유수 free water가 내이 외림프로 새어들어 간다. 그래서 혈-림프 격막의 누출은 와우 기능 장애와 병리 변화에 영향 끼친다. 주위혈액 공간에 존재하는 대식세포가 혈-외림프 격막을 유지하는데 관여한다는 연구가 있지만[18], 어떤 세포가 이 격막의 기능 장애를 초래할 지에 대한 명확한 근거는 부족한 현실이다.

미로막 격막에 대한 연구는 다양한 약물의 약물 동력학에도 많은 연관이 있어 연구가 진행되고 있는데, 일부 약물의 조합은 미로막 격막을 통한 투과력을 증가시켜 잠재적으로 치료 약물의 내이로의 통과를 유도 할뿐만 아니라 이독성을 증가시킬 수 있다는 보고가 있다[19]. 그래서 내이의 지속적 전달 시스템과 관련하여 다양한 약물의 개선 된 치료 효능 및 부작용 감소를 제공 할 수 있는 임상 적용을 위한 동물 모델 연구 또한 필요하다.

3. 유전 Gene

메니에르병은 내이의 만성질환이며, 대부분은 산발성으로 발생한다. 하지만 5%–15%에서는 가족력을 보이고, 메니에르 환자의 형제 자매가 이 질병에 이환될 위험이 10배 증가 소견을 보여[20], 메니에르병은 아마도 유전적 요인과 환경적 요인의 상호작용으로 인해 발생하는 다인자 multifactorial 질환으로 여겨진다. 메니에르병의 유전성은 1941년부터 기술되어왔다[21]. 그 이후로 상 염색체 우성 유전의 가능성을 뒷받침하는 여러 보고가 있었지만 불완전한 침투율(incomplete penetrance)은 약 60%로 추산된다[22-24]. 최근엔 매우 큰 집단에서 상 염색체 열성 유전체가 있는 일부 가족이 보고 되었다[25].

아직 메니에르병의 병인이 명확하지 않기 때문에, 발병기전을 유전학적으로 밝히려는 연구가 활발히 이루어 지고 있는데(Table 1) [7,26-49], 메니에르병의 유전적 배경을 뒷받침하는 몇 가지 특성이 있다. 메니에르병의 인종 감수성은 문헌에 잘 기록되어있는데, 여성(56%)이 남성보다 더 잘 발생되고 질병 발병 연령은 거의 동일하다[50]. 아프리카 계통의 개체에서는 극히 드문 질병으로 민족적 편향성을 나타내며, 일본인의 발병률[51]은 1/3,500인 반면 백인의 발병률[52]은 약 1/2,000으로 추정한다[53].

Reports on genetics studies on Ménièreʼs disease

Requena 등[54]은 전체 엑솜 시퀀싱을 통해 2번 염색체에 위치한 FAM136A 유전자와 18번 염색체에 위치한 DTNA 유전자에서 2개의 새로운 이형 접합체 변종(heterozygous single-nucleotide variants)을 발견했으며, 상염색체 우성 유전된다고 보고하였다. 최근 한국인에서도 가족성 메니에르병의 빈도가 약 6.3%로 보고되고 있다[55].

보고된 유전자들 중에서도 와우내 전위 endocochlear potential 유지를 위해 내이에서 이온 항상성의 중요성을 고려하여 림프액 및 이온 항상성과 연관된 유전자들도 발견되고 있다. 이 중 potassium channels genes인 KCNE1, KCNQ1 채널의 돌연변이는 내이 및 심장에서 동시에 발현되어 심한 감각 신경성 난청을 일으키고 와우관 중간계 scala media 붕괴 소견을 나타낸다[56].

어떤 연구에서는 유전체 연관분석을 통해 특정 단일염기다형성인 SIK1 유전자(rs3746951,)과 SLC8A1 (rs487119)가 메니에르병 환자들에서 유의 있게 관찰되었다[57]. SLC8A1 유전자는 Na–Ca 교환을 부호화하고, SIK1 유전자는 Na–K ATPase와 결합하는 효소인 Salt inducible kinase 1을 부호화한다. 이런 결과는 유전적으로 유도된 이온 수송장애가 메니에르병을 유발할 수 있는 선행 요인으로 작용할 수 있다는 가설을 뒷받침하고 있다.

요약하면, 지금까지의 유전자의 접근법에 기초한 연구는 메니에르병 발병과 관련된 명백한 유전자는 아직 제공하지 못하고 있지만, 다형성과의 연관성에 기반을 둔 유전자 연구는 질병의 발달에 대한 더 많은 정보를 제공 하고 있다. 메니에르병의 발병 유전자를 발견하기 위해서는 대상 환자의 선택이 매우 중요함으로 산발성 환자들보다는 명확한 기준에 따라 진단된 가족성 메니에르병 환자들을 우선적으로 고려해야 하겠다.

4. 자기공명영상

내림프수종은 메니에르병의 최종 병리학적 소견이다. 어떤 환자의 내이 증상들이 메니에르 진단 지침과 일치하지는 않지만 내림프수종과 관련이 있을 것으로 생각한다. 내림프수종과 메니에르병과의 상관관계에 대해 논란이 있다. 1938년부터 2012년까지의 Medline을 통한 포괄적 문헌 고찰에 의하면 내림프수종이 메니에르병의 원인이 된다면 수종 단독으로는 메니에르병을 일으키기에 불충분하며, 무증상의 내림프수종이 증상이 있는 메니에르병으로 발병케 하는 하나 이상의 추가적 보완 요소가 있음이 보고 되었다[58].

내림프수종은 내림프 공간을 경계로 하는 구조가 내림프 부피의 확장으로 인해 팽창되는 병적인 해부학적 소견이다. 확실한 메니에르병 진단은 사후 측두골 표본에서 내림프 수종의 조직학적 증명으로만 확인될 수 있다. 따라서 내림프수종의 진단은 전정기능검사와 임상 증상을 기반으로 이루어졌다. 최근 MRI의 발전으로 내림프 수종의 객관적인 진단이 가능해지고 영상화 방법, 평가 방법, 영상화 결과와 전정 기능 이학적 검사의 상관 관계 및 영상화 소견과 임상 증상의 상관 관계에 대한 많은 연구 결과가 보고되었다[59-63].

높은 칼륨과 낮은 나트륨 농도를 갖는 내 림프의 조성은 세포질의 액상 조성과 유사하고 낮은 칼륨과 높은 나트륨 농도의 외림프의 조성은 세포외액의 조성과 유사하다. 그러나 이러한 화학적 조성의 차이에도 불구하고 그리고 내림프와 외림프를 분리하는 Reissner 막이 너무 얇아서[64] 내외림프 공간을 영상으로 시현하기는 쉽지 않다. 그래서, 고실내 조영제 투여후 자기공명 촬영기법을 이용하고 있다. 고실내 투여된 가돌리움 조영제는 난원창과 정원창을 통해 미로의 외림프 공간으로 확산되고, 와우관 중간계(cochlear duct, 내 림프 공간)은 촬영된 MRI 영상에서는 음영결손(filling defect)으로 시각화 된다[65]. Nakashima 등[64]은 3차원 3D fluid attenuated inversion recovery (FLAIR)을 이용하여 희석된 가돌리움 조영제를 메니에르 환자의 고실내로 투여하여 내림프 수종의 선명한 영상을 처음으로 보고하였다. 고실내 주입 된 조영제의 부피는 대개 0.3–0.6 mL이며, 환자는 주사 후 약 30분 동안 그대로 누워 있어야 되며, MRI은 일반적으로 고실내 조영제 투여 24시간 후 수행하게 된다. 반면에 조영제 정맥투여 시는 4시간 후에 촬영을 한다. 고실내 조영제 투여후 얻은 3D-FLAIR 영상에서, 외림프는 희석된 가돌리움 조영제의 분포로 인해 고신호 강도를 보이고, 대조적으로, 희석된 가돌리움 조영제가 없는 내림프는 뼈와 공기와 비슷한 저신호 강도를 나타낸다[66-68].

Yoshida 등[69]은 메니에르병 환자의 고실내 가돌리움 조영제를 이용한 MRI 연구에서 와우 내림프수종 소견이 메니에르병 환자에서 87%, 정상귀에서 38% 관찰되었고, 전정 내림프수종은 메니에르병 환자에서 94%, 정상귀에서 0% 관찰되었다. 명확한 메니에르병 진단에 MRI에서 전정 내림프수종 관찰은 민감도 및 특이도가 각각 88.5%, 100%로 나타내 진단에 유의하게 도움이 된다고 보고하였다. Tunon Gomez 등[70]은 메니에르병 환자와 면역 매개 내이질환 의심 환자를 대상으로 MRI를 통해 내림프수종 소견을 관찰하였고, 내이질환 진단 시 MRI 촬영이 필요하다고 보고 하였다.

메니에르병의 만성적 어지럼증과 청력 장애는 대부분 일상생활과 정서적인 스트레스를 유발한다. 이 스트레스는 새로운 신체 항상성을 생성하는 기전인 알로스타 부하를 유도하고 시상하부-뇌하수체-부신 축에 의한 코르티졸의 지속적인 분비를 초래하여 개인을 질병에 더 취약하게 만든다. 메니에르병 환자의 만성 스트레스 노출 정도를 MRI 영상을 이용해 간접적으로 반영 할 수 있는 해마 체적간의 관계를 밝힌 연구에서, 메니에르병 환자는 해마 체적이 유의하게 감소되어 있었고, 부피는 청력 및 전정 기능의 정도와 유의한 상관 관계가 있었다. 이것은 예측할 수 없이 불현듯 발생하는 어지럼과 청력 소실에 노출된 메니에르병 환자는 만성 스트레스가 쌓이게 되고, 결국 스트레스 관련 호르몬의 증가로 인해 해마 위축을 유발할 수 있다는 것이다[71].

많은 이비인후과 의사들은 스트레스와 메니에르병 간에 밀접한 연관이 있을 것이라는 생각을 갖고 있다. 메니에르병은 스트레스가 많은 일상 활동에 참여하거나 스트레스가 많은 사회 환경 내에서 상호 작용함으로써 정신적 또는 신체적 스트레스에 의해 유발될 수 있어 메니에르병에 대한 심리학적 및 인지적 치료법에 대해 소개 되고 있다[72]. 그래서 정신적, 신체적 스트레스와 스트레스 호르몬인 바소프레신-AQP 2 시스템의 활성화로 인해 내림프수종이 유발되고, 바소프레신 길항제의 탈수 효과로 인해 내림프수종의 발생이 감소된다는 연구가 있고[73], 바소프레신 분비관리를 통해 메니에르병을 치료하기 위한 새로운 치료적 개입 방법이 소개 되었다[74].

결 론

메니에르병은 현기증, 청력 변동, 이명 및 이충만감을 유발하지만 아직 여전히 잘 이해되지 않는 증후군이다. 저자는 최근 메니에르병의 조직 병리학, 세포 화학, 유전, 미로막 격막 및 영상화에 대해 살펴보았다. 조직 병리학은 유모세포 손실, 기저막 막 비후 및 혈관 주위 미세 혈관 손상으로 인한 신경 상피 손상에 중요하고, 세포 화학적 변화는 지지 세포에서의 AQP4 및 AQP6 발현의 변화 및 코클린 및 미토콘드리아 단백질 발현의 변화에 중요하다. Aquaporin 채널(AQP1, AQP2, AQP3, AQP4, AQP5, AQP6, AQP7 및 AQP9)이 발견 된 이래로 이들 채널이 내이 유체 항상성에 중요한 역할을 한다는 것이 밝혀지고 있다. 메니에르병의 발병 유전자를 발견하기 위해서는 산발성 환자들보다는 명확한 기준에 따라 진단된 가족성 메니에르병 환자들을 우선적으로 고려해야 하겠다. 또한 메니에르병 의심환자에서 향상된 MRI을 이용해 내림프 수종의 정도와 존재를 평가하는 영상 기술이 앞으로 많이 발달되어, 향후 내림프수종의 여부와 임상 소견을 연관 지을 수 있는 연구가 필요할 것이다.
