  • Objectives
    Vertigo combined with sudden sensorineural hearing loss (SSNHL) is known as a poor prognostic factor. We investigated clinical findings and vestibular function tests in patients of SSNHL with vertigo to find the prognostic factors.
  • Methods
    We retrospectively evaluated data on the patients diagnosed with SSNHL with vertigo at Bucheon Soonchunhyang University Hospital from March 2009 to February 2018. We reviewed medical records and the results of vestibular function tests and audiometry.
  • Results
    Of the 68 patients, 30 (44.1%) showed profound hearing loss and 53 (77.9%) showed poor recovery. Age and the degree of initial hearing loss showed negative prognostic factor in hearing recovery. Abnormal results of cervical vestibular evoked myogenic potentials (cVEMP) also showed significantly differences between good and poor recovery groups.
  • Conclusions
    In this study, most of the patients of SSNHL with vertigo showed poor recovery. Age, degree of initial hearing loss, and the abnormal result of the cVEMP have a negative effect on the prognosis of hearing recovery.

돌발성난청은 일년에 10만 명당 5–20명 정도에서 특별한 원인 없이 갑자기 발생하는 감각신경성난청으로 이명과 현훈 등의 증상을 동반하기도 한다. 발병 원인이나 기전에 대해서는 아직 명확하게 밝혀지지 않았으나, 바이러스 감염, 혈관 장애, 내이질환 등이 원인으로 생각되고 있다[1,2].
일반적으로 돌발성감각신경성난청 환자의 자연 회복률은 약 32%–65% 정도로 보고되고 있다[2,3]. 하지만, 소아나 40세 이상의 연령, 현훈, 고음역의 청력 소실, 어음 명료도의 저하 등이 동반된 경우에는 예후가 불량한 것으로 알려져 있다[2,4,5].
특히, 돌발성감각신경성난청 환자의 30%–40%는 현훈을 호소하는데, 현훈이 동반되는 경우 청력소실이 심한 경우가 많고, 현훈이 없는 돌발성감각신경성난청 환자와 비교하면 회복률이 좋지 않은 것으로 알려져 있다[6]. 예후가 나쁘다고 알려진 현훈을 동반한 돌발성감각신경성난청 환자에서 온도안진검사, 양성돌발두위현훈, 경부 전정유발근전위(cervical vestibular evoked myogenic potential) 등 다양한 요인이 청력회복에 영향을 미친다고 보고자마다 다르게 보고되고 있다[7-9]. 따라서 이번 연구는 현훈을 동반한 돌발성감각신경성난청 환자에서의 전정기능검사 결과와 예후와의 연관성을 알아보고자 하였다.
1. 대상
2009년 3월부터 2018년 3월까지 본원 이비인후과에 내원하여 급성돌발성감각신경성 난청으로 진단받고 치료받은 환자 중에서 현훈을 동반한 환자를 대상으로 후향적 의무기록을 분석하였다. 돌발성난청의 과거력이 있거나, 중추성현훈이나 메니에르병이 의심되는 경우, 중이염이 있는 경우, 내원하여 시행한 자기공명영상에서 전정신경초종이 있는 경우는 대상에서 제외하였다.
2. 방법
현훈을 동반한 급성돌발성감각신경성 난청 환자에서 입원 당시 측정한 순음청력검사, 자발안진검사, 체위성안진검사, 온도안진검사, 경부 전정유발근전위검사 등을 분석하였다.
순음 청력검사는 500, 1,000, 2,000, 4,000 Hz에 대한 주파수별 청력손실을 4분법으로 평균청력역치를 계산하였다. 이환 초기의 난청의 정도는 Hong 등[10]이 분류한 방법에 따라 grade 1 (26–55 dB), grade 2 (56–90 dB), grade 3 (91 dB 이상)로 분류하였다. 2개월간의 추적 관찰 후 청력회복을 평가하였는데, 청력회복은 Sigel’s criteria를 기준으로 25 dB 역치 이내로 청력 호전이 있는 경우 완전회복(complete recovery), 15 dB 이상 청력 호전이 있고 최종 25–45 dB 범위의 청력 역치인 경우를 부분회복(partial recovery, PR), 15 dB 이상 청력 호전이 있고 최종 45 dB 이상 범위의 청력 역치인 경우를 경도회복(slight recovery), 15 dB 미만으로 청력 호전이 있거나 최종 75 dB 이상 범위의 청력 역치인 경우 불변(no improvement)로 구분한 뒤, 완전회복과 부분 회복을 좋은 회복(good recovery), 경도회복과 불변을 나쁜 회복(poor recovery)로 보았다[11].
대상 환자들은 외래에서 frenzel goggle (Nagashima Medical Instruments, Tokyo, Japan)을 이용하여 자발안진검사와 체위성안진검사를 시행하여 자발안진 유무와 양성돌발두위현훈 동반 여부를 확인하였다.
온도안진검사는 환자를 60° 눕힌 상태에서 외이도에 섭씨 24°C와 50°C의 공기로 1분간 온도자극을 주었으며, Jonkee 공식을 이용하여 반고리관 마비를 계산하였다. 반고리관 마비(canal paresis)가 25% 이하를 정상, 초과하였을 때를 비정상으로 해석하였다.
경부 전정유발근전위검사는 90 dB의 클릭음을 헤드폰으로 자극하였고, p13과 n23의 잠복기와 진폭차의 비를 구하여 본원 검사실 정상치와 비교하였다. 파형이 없거나 양이 진폭차의 비(vestibular evoked myogenic potential asymmetry)가 27.7을 초과했거나, p13의 잠복기가 15.07 msec을 초과했을 때 비정상으로 하였다[12].
통계는 IBM SPSS Statistics ver. 20.0 (IBM Co., Armonk, NY, USA) 프로그램을 이용하여 환자의 임상 양상 및 전정 기능의 손상 정도와 청력회복과의 관계는 Student t-test 와 chi-square test 또는 Fisher exact test를 통해 검증하였다. 통계학적 유의수준은 p-value 0.05 미만으로 하였다.
총 68명의 환자를 이번 연구에 포함하였다(Table 1). 이들의 평균 연령은 49.46±15.17세였고, 성별에 따라 남자가 35명(51.5%), 여자가 33명(48.5%)이었다. 이환 초기 청력소실 정도는 grade 1이 14명(20.6%), grade 2가 24명(35.3%), grade 3가 30명(44.1%)이었다. 자발안진이 15명에서 관찰되었고, 온도안진검사상 비정상은 27명(39.7%)이었고, 경부전정유발근전위검사에서 비정상 소견은 46명(67.6%)이었다. 또한 7명에서 병변측에 양성돌발두위현훈이 동반되었는데, 5명은 후반고리관결석증, 2명은 수평반고리관결석증이었다.
최종 회복 정도를 분석했을 때, 15명은 좋은 회복, 53명은 나쁜 회복에 해당하였다(Table 2). 좋은 회복군의 평균 연령은 41세로 나쁜 회복군의 평균 연령인 52세보다 유의하게 낮았으며, 두 군 간의 초기 청력소실 정도는 나쁜 회복을 보인 군에서 보다 높은 경향성을 보였고, 또한 나쁜 회복을 보인 군에서 비정상적인 경부 전정유발근전위검사가 흔했다. 두 군 간에 환자의 연력 및 초기 청력소실 정도, 경부 전정유발근전위검사 결과는 통계적으로 유의한 차이를 보였다. 그 외, 성별, 병변의 위치, 자발안진과 양성돌발두위현훈 유무, 온도안진검사 결과는 유의한 차이가 없었다.
돌발성난청은 1944년 De Kleyn [13]이 처음 보고한 이후 현재까지도 정확한 원인이나 예후에 대해 밝혀지지 않았다. Noury와 Katsarkas [14]은 돌발성난청에서 현훈이 동반되면 청력소실 정도가 농이거나 고음역 청력소실이 동반되는 경우가 많다고 하였고, Nakashima와 Yanagita [15]의 연구에서도 비슷한 결과를 보였다. 이 연구에서는 현훈을 동반한 돌발성감각신경성난청 환자를 대상으로 연구를 진행하였다. 전체 환자 68명의 환자 중에서 91 dB 이상의 청력소실을 보이는 환자가 30명(44.1%)이고, 나쁜 회복을 보이는 경우가 53명(77.9%)이었는데, 현훈이 동반된 경우 초기 청력소실 정도가 심한 경우가 많고, 나쁜 예후를 보이는 기존의 연구 결과와 유사하였다.
예후에 있어서 초기의 청력소실 정도가 연관성이 있다고 알려져 있다. Sheehy [16]은 초기의 청력손실이 클수록 예후가 나쁘다고 보고하였다. Hong 등[17]의 연구에서 초기 청력소실이 100 dB가 넘는 전체 환자에서 완전회복을 보인 환자는 한 명도 없었고, 단 두 명만이 부분회복을 보였다. 이 연구에서도 이환 초기의 청력소실 정도가 좋은 회복과 나쁜 회복을 보인 두 환자군에서 유의한 차이를 보였으며 고도난청을 보인 환자들이 많은 영향이라고 생각한다.
양성돌발두위현훈이 동반된 돌발성난청이 청력회복에 미치는 영향에 대해서는 아직 논란이 많다. Lee와 Ban [18]은 돌발성난청 환자에서 동반된 양성돌발두위현훈은 미로의 손상을 보이는 것으로, 돌발성난청의 나쁜 예후 인자라고 보고하였다. 반면, Kim 등[19]은 양성돌발두위현훈이 동반된 돌발성난청 환자가 양성돌발두위현훈이 없는 돌발성난청 환자와 유의한 차이를 보이지 않았고, 양성돌발두위현훈이 주요 예후 인자로 볼 수 없다고 보고하였다. 이 연구에서도 양성돌발두위현훈은 치료 후 회복과 유의하지 않은 상관관계를 보였다.
현훈을 동반한 돌발성감각신경성난청 환자에서의 전정신경검사와 청력회복에 대한 다양한 연구들이 보고되었다. Iwasaki 등[20]은 연구를 통해 온도안진검사와 전정유발근전위검사의 비정상 소견과 청력회복과의 관계에 대하여 보고하였다. 현훈을 동반한 돌발성감각신경성난청 환자의 45%에서 온도안진검사상 이상 소견을 보였고, 77%에서 전정유발근전위검사상 파형의 소실을 보였는데, 온도안진검사상 이상을 보인 환자 중 50% 이상 청력회복을 보인 환자는 20%밖에 보이지 않았다. Shih 등[21]의 연구에서는, 현훈을 동반한 돌발성난청환자 중, 온도안진검사상 비정상 반응을 보인 경우, 정상 반응을 보인 경우보다 청력회복이 나쁜 경우 통계적으로 유의하게 많음을 보고하였다. 특히, 돌발성난청의 예후와 전정유발근전위검사와의 연관성에 대한 연구들을 살펴보면, Hong 등[10]의 연구에서는 전정유발근전위 반응의 비정상 파형과 청력회복의 정도는 유의한 차이를 보이지 않았으나, Yun 등[22]의 연구에서는 전정유발근전위검사상 정상과 비정상 두 군 간 청력회복의 차이를 보여 예후에 영향을 미치는 것으로 나타났다. 이 연구에서는 온도안진검사상에서는 유의하지 않았으나, 나쁜 회복을 보인 환자군에서 경부 전정유발근전위 반응의 비정상 소견을 보인 비율이 73.6%로 좋은 회복을 보인 환자군과 비교하여 통계학적으로 유의하게 높은 수치를 보였다. 구형낭과 와우의 해부학적 위치가 가까워서 소리 에너지 전달과정에서 동시에 손상이 발생 가능하다는 점과 둘 다 총와우동맥(common cochlear artery)에서 혈액공급을 받는다는 점과 연관이 있다고 생각된다[23,24].
이 연구의 한계점으로는 현훈을 동반하지 않은 군을 조사하여 비교하지 않은 점, 증례 수가 많지 않다는 점, 환자의 전정 기능을 평가하는 데 있어 수평반고리관과 구형낭에 국한되어 있다는 점이 있다. 또한, 환자의 청력도의 양상에 대한 연구가 이뤄지지 않아 청력도 양상과 돌발성감각신경성난청의 예후와의 관계를 확인할 수 없었다.
결론적으로, 현훈이 동반된 돌발성감각신경성난청 환자에서 대부분이 나쁜 청력 회복을 보였고, 환자의 나이, 이환 초기 청력소실 정도, 경부 전정유발근전위검사상 비정상 파형을 보인 경우에서 청력회복에 나쁜 예후를 보였다.
