Abstract

Objectives:
This study was performed to evaluate the difference of the cervical vestibular evoked myogenic potentials (cVEMP) stimulated by bone-conduction (BC) and air-conduction (AC) in patients with nonspecific dizziness.
Methods:
Twenty-eight dizzy patients (56 ears) and 15 subjects (30 ears) as normal control was enrolled. Responses of BC- and AC-cVEMP were recorded sequentially in both groups. cVEMP parameters including latencies, inter-latencies intervals, amplitudes, and interaural amplitude asymmetry were analyzed and compared.
Results:
Among the patients with nonspecific dizziness, AC-cVEMP responses were clearly detected in all 56 ears while BC-cVEMP responses were detected in 32 ears (57.1%). Amplitudes of BC-cVEMP were significantly smaller than those of AC-cVEMP in all patients with BC-cVEMP response. There was no difference in latencies, inter-latencies intervals, and interaural amplitude asymmetry ratios between BC- and AC-cVEMP. There was no significant difference in BCand AC-cVEMP between the dizzy and control groups.
Conclusions:
BC-cVEMP is not clinically useful in comparison to AC-cVEMP for the evaluation of nonspecific dizziness. An effective stimulation tool for BC is necessary to provoke more reliable responses of BC-cVEMP.

서 론

경부전정유발근전위(cervical vestibular evoked myogenic potentials, cVEMP)는 소리 자극에 의해 내이의 구형낭(saccule)이 반응하여 하전정신경(inferior vestibular nerve)을 통해 동측의 흉쇄유돌근(sternocleidomastoid muscle)이 수축하는 반응을 기록하는 검사로서, 국내 여러 병원에서 임상적으로도 많이 이용되고 있을 뿐만 아니라 이석기관 기능 평가를 위한 연구에서 다양하게 확인되고 있다[1-4]. 그럼에도 불구하고 기도(air-conduction) 자극에 의한 cVEMP는 다수의 이비인후과 및 신경과 검사실에서 널리 시행 중인 반면, 골도(bone-conduction) 자극에 의한 cVEMP는 반응이 잘 안 나오거나 약하게 나오기 때문에 연구 목적 이외에는 별로 활용되지 못하고 있다[2].

전음성 난청 환자와 정상 대조군에서는 골도 자극음을 주었을 때 cVEMP 반응이 잘 나오고, 감각신경성 난청 환자나 전정신경 절제술(vestibular neurectomy)을 시행 받은 환자에서는 골도 cVEMP 반응이 나오지 않는다는 점[5]에서 골도 cVEMP도 기도 cVEMP와 마찬가지로 전정에서 기원하지만 골도 청력이 관여함을 확인할 수 있다. 고막 천공이나 중이염에 의한 전음성 난청에서 기도 청력이 떨어져 있으면서 골도 청력은 보존되어 있는 환자의 경우, 기도 cVEMP에 비해 골도 cVEMP가 더 반응이 안정적으로 잘 나오고 특이성(specificity)도 비교적 높은 수준으로 보고된 바 있다[6,7]. 기도 cVEMP와 더불어 골도 cVEMP의 장점도 존재하지만, 어지럼증 환자들을 대상으로 골도와 기도 cVEMP를 모두 시행하여 그 결과들을 직접 비교-분석한 국내 연구는 거의 없는 상황이다.

이에 본 연구에서는 비특이적인 어지럼증을 동반한 환자들과 정상 대조군에서 골도 cVEMP 반응이 나타나는 비율(incidence)을 알아보고, 동일한 대상자 내에서 기도 cVEMP의 지표들과 비교하여 골도-기도 cVEMP 검사 간에 차이가 있는지를 조사해보고자 하였다.

대상 및 방법

이 연구는 본 병원에 소속된 기관윤리심의위원회의 심의를 통과하였다(EMC IRB 14 – 47).

본 병원 이비인후과에 어지럼증을 주소로 내원하여 진찰 및 검사를 받은 결과 특이 소견이 없는 환자 28명(56개 귀; 평균 나이, 52.4±13.7세[범위, 28–71세]; 남성:여성, 12:16)과 아무 증상이 없는 정상 대조군 15명(30개 귀; 평균 나이, 51.8±12.6세[범위, 26–67세]; 남성:여성, 7:8)을 모집하였다. 모든 대상자에서 귀 내시경검사, 비디오 안진검사(자발안진, 주시유발안진, 체위안진, 두진후안진), 순음 청력검사, 골도 및 기도 전정유발근전위검사를 시행하였으며, 다른 내이 질환을 배제하기 위해 귀 내시경검사에서 고막에 이상 소견이 있거나, 순음청력검사에서 골도 및 기도 각각 25 dB 이내의 정상 청력이 아닌 경우, 비디오 안진검사에서 안진이 나타나는 경우는 연구 대상에서 제외하였다. cVEMP는 기도 자극으로 먼저 검사하였고, 같은 날 이어서 골도로 자극하여 cVEMP 반응을 평가하였다.

cVEMP 검사는 대상자가 누운 자세에서 방음 환경 하에 측정하였고, 활성 전극을 흉쇄유돌근의 중간 부위에, 기준 전극을 흉골의 상부에, 접지 전극을 이마 중앙에 붙이고, 자극하는 귀의 반대편으로 고개를 돌린 후 일정한 힘을 유지하도록 하면서 시행하였다. 이 때, 별도의 흉쇄유돌근 수축을 확인하거나 normalized vestibular evoked myogenic potential (VEMP) peak을 사용하지는 않았다. 전극 부착 전에 저항 억제 젤(impedance lowering gel)을 부착 부위의 피부에 도포하였고, 피부의 저항은 5 kΩ 이내로 확인되었다. 기도 자극에 의한 cVEMP는 500 Hz 톤 버스트(tone burst)를 90 dB normal hearing level (nHL) 강도로 ER-3 삽입형 수화기(insert phone)를 통해 초당 5.1회로 시행하였고, 골도 자극에 의한 cVEMP는 동일한 자극음을 60 dB nHL 강도로 귀 뒤의 유양돌기(mastoid process)에 부착된 B-71 (Radioear, New Eagle, PA, USA) 골도 진동자(bone vibrator)를 통해 검사하였다. 골도 및 기도 자극음은 최대 진폭으로 오르는 상승 시간(rise time)과 다시 영점으로 떨어지는 하강 시간(fall time)이 각각 1 msec였고, 자극음의 최대 진폭이 일정하게 지속되는 유지 시간(plateau time)은 2 msec였다. cVEMP 신호는 10–1,000 Hz로 필터링하였고 최대 120회의 반응을 평균하여 기록하였으며, 검사 기기는 Bio-logic Navigator Pro Evoked Potential System(Natus Medical Inc., San Carlos, CA, USA)을 이용하였다.

최대 자극에서 나타난 파형 중 13 msec 근처의 양전위인 p13, 23 msec 근처의 음전위인 n23에 대해 두 개의 파형의 잠복기(latency)와 진폭(amplitude)을 측정하였고, 두 파형의 잠복기 간의 차이인 파간 잠복기 차이(inter-latencies interval)와 양이간 진폭 비대칭성(interaural amplitude asymmetry)을 계산하였다. 진폭은 p13과 n23의 최대 진폭 차이를 먼저 구한 후 검사 기기에 프로그래밍 되어있는 prestimulus rectification function을 통해 자동으로 계산되었다[8]. 양이간 진폭 비대칭성은 p13과 n23의 최대 진폭 차이를 계산하여 좌우 진폭의 차를 그 합으로 나눈 값([우측 진폭차–좌측 진폭 차]/[우측 진폭 차+좌측 진폭 차]×100%)으로 정의하였다. 따라서, 양이간 진폭 비대칭성은 양의 값이면 우측으로의 진폭 비대칭을, 음의 값이면 좌측으로의 진폭 비대칭을 의미한다.

모든 통계 분석은 SPSS ver. 12.0 (SPSS Inc., Chicago, IL, USA)을 이용하였다. cVEMP의 잠복기, 파간 잠복기 차이, 진폭, 양이간 진폭 비대칭성의 평균 비교에는 paired t-test를 사용하였고, 골도와 기도의 양이간 진폭 비대칭성에 대한 상관관계는 Spearman correlation coefficient로 통계적인 유의성을 분석하였다. 환자군과 정상 대조군 간 검사 결과의 평균은 Mann-Whitney U-test로 비교하였다. 각각의 변수들은 평균±표준편차의 형식으로 기술하였고, 유의수준은 0.05 미만을 의미가 있다고 해석하였다. 각각의 변수는 평균±표준편차의 형식으로 기술하였다.

결 과

1. 임상 양상과 골도 및 기도 경부전정유발근전위의 발생률 비교

비특이적인 어지럼증 환자군 28명의 순음청력검사 상 500, 1,000, 2,000, 3,000 Hz의 주파수에서 4분법으로 계산한 순음청력 평균(pure tone average)은 우측 13.8±10.3 dB(범위, 3–24 dB), 좌측 15.2±9.7 dB (범위, 4–25 dB)이었다. 어지럼 증상은 회전성:비회전성(16:12), 급성:만성(13:15), 단발성:반복성(7:21), 자발성:유발성(20:8)으로 다양했고, 지속 시간도 수 초에서 수 일로(평균, 수 시간)로 다양한 양상을 보였다. 정상 대조군 15명의 순음청력 평균은 우측 11.4±8.6 dB (범위, 0–22 dB), 좌측 10.9±9.1 dB (범위, 2–24 dB)이었다.

기도 cVEMP 검사는 환자군 56개 귀, 대조군 30개 귀 모두에서 반응이 명확하게 관찰된 반면, 골도 자극을 이용한 cVEMP에서는 환자군 32개 귀(57.1%), 대조군 16개 귀(53.3%)에서만 반응이 확인되었다(Table 1). 모든 환자군 56개 귀의 기도 cVEMP 검사 상, 우측 p13, n23 파형 잠복기의 평균은 각각 16.7±1.3 msec, 25.4±1.7 msec이였고, 좌측 p13, n23 파형 잠복기의 평균은 각각 16.4±1.2 msec, 26.1±1.5 msec이였다. 골도 cVEMP 반응이 확실하게 나온 16명과 반응이 명확히 나타나지 않은 12명의 환자들 간에 유의한 인구학적, 청각학적인 차이는 보이지 않았다(p＞0.05) (Table 2).

2. 골도 및 기도 경부전정유발근전위의 잠복기 비교

비특이적인 어지럼증 환자군 중 골도와 기도 cVEMP 반응이 모두 나타난 16명 32개 귀에 대해 분석하였을 때, 기도 cVEMP의 경우 p13, n23 파형의 평균 잠복기는 각각 16.6±1.4 msec, 25.6±1.7 msec, 골도 cVEMP에서 p13, n23 파형의 평균 잠복기는 각각 18.3±2.1 msec, 26.5±1.9 msec으로 두 검사 간에 유의한 차이를 보이지 않았다. 정상 대조군 중 골도와 기도 cVEMP 반응이 모두 나타난 8명, 16개 귀에서도 골도-기도 cVEMP 잠복기의 평균 간에 유의한 차이를 보이지 않았다. 어지럼증 환자군의 평균 파간 잠복기 차이는 기도 cVEMP에서 ‒9.2±5.8 msec, 골도 cVEMP에서 ‒7.5±2.1 msec로 기도-골도 두 검사 간에 유의한 차이를 보이지 않았다(p＞0.05).

3. 골도 및 기도 경부전정유발근전위의 진폭 비교

비특이적인 어지럼증 환자군 중 골도와 기도 cVEMP 반응이 모두 나타난 32개의 귀에서 prestimulus rectification function에 의해 계산된 진폭의 평균은 기도 cVEMP에서 12.1±5.8 μV, 골도 cVEMP에서 6.9±3.1 μV로, 골도 자극에 의한 cVEMP 반응의 진폭이 기도 자극에 비해 통계적으로 의미 있게 작았다(p＜0.001). 정상 대조군 중 골도와 기도 cVEMP 반응이 모두 나타난 16개 귀에서도 골도 cVEMP 진폭의 평균은 6.7±3.4 μV로, 기도 cVEMP의 11.5±6.2 μV에 비해 유의하게 작게 나타났다(p＜0.001). 어지럼증 환자군의 평균 양이간 진폭 비대칭성은 기도 cVEMP가 0.2%±12.3% (범위, ‒24% to +28%), 골도 cVEMP가 0.5%±19.6% (범위, ‒20% to +32%)로 기도-골도 검사 간에 유의한 차이를 보이지 않았다(Table 3). 환자군과 정상군 모두 골도와 기도 cVEMP의 양이간 진폭 비대칭성은 서로 유의한 상관관계를 보이지 않았다(p＞0.05).

고 찰

내이의 여러 구조물 중에 와우(cochlea)는 청각을, 전정은 평형 감각을 담당하지만, 인간에서도 중이에서 내이로 소리를 전달하는 등골(stapes)이 와우가 아닌 전정의 일부분인 구형낭 부근에 부착되어 있고, 하등 척추동물 및 어류의 일부에서는 여전히 구형낭이 청각 기관으로 작용하고 있음을 통해 구형낭이 와우 이전의 진화 단계에서 청각기관으로서 소리에 대한 반응을 담당했음을 추론해 볼 수 있다[9,10]. 기도와 골도 소리 자극에 의해 cVEMP 반응이 나타난다는 점 또한 이러한 추론을 뒷받침해주는 근거 중의 하나로 판단된다. 즉, 구형낭 및 하전정신경에서 기원하는 불수의적인 cVEMP 반응을 통해 소리 자극에 대한 반사(sacculocollic reflex to sound)로서 목을 자동적으로 움직일 수 있게 되는 것이다. 안구 근육의 전정유발 반응을 확인하는 ocular VEMP는 cVEMP와 유사하게 소리 자극에 대한 전정 반응이라는 임상적 의미를 지니지만, 전정 중에서 구형낭 대신 난형낭(utricle)에 의해, 하전정신경 대신 상전정신경(superior vestibular nerve)에 의해 나타난다는 점에서 차이점이 있다(utriculo-occular reflex to sound) [11]. 기도와 골도 소리 자극으로 구형낭과 난형낭이 모두 자극을 받을 수 있지만, 흉쇄유돌근에서 관찰되는 cVEMP 반응은 구형낭을 통한 전정-경부반사에 의해 주로 유발되기 때문에 cVEMP는 구형낭과 하전정신경의 기능을 확인하는 검사로 알려져 있다.

골도 자극에 의한 cVEMP 반응의 발생률은 2000년 Sheykholeslami 등[5]이 골도 클릭음과 짧은 톤 버스트(short tone bursts)를 이용해 20명의 정상 지원자와 12명의 전음성 또는 감각신경성 난청 환자에서 나타남을 보고한 이래, 대상자의 청력 상태, 자극 방법이나 골도 진동자의 종류에 따라 0%에서 100%로 다양하게 나타나고 있다[2,5,12-15]. 본 연구에서는 골도 뇌간청성반응(auditory brainstem response) 검사에 보편적으로 사용되고 있는 B-71 골도 진동자를 통해 500 Hz의 톤 버스트 자극음을 60 dB nHL 강도로 자극하였을 때, 비특이적인 어지럼증 환자 56개 귀와 정상 대조군 30개 귀에서 각각 57.1%와 53.3%의 골도 cVEMP 반응이 명확하게 관찰되었다. 이는 B-71 골도 진동자에 의해 정상 청력을 보이는 74개 귀의 55.4%에서 골도 cVEMP 반응이 확인된 Kang 등[2]의 연구와 유사하지만, modified BR71 (Rion, Tokyo, Japan) 골도 진동자를 통해 125–4,000 Hz의 톤 버스트 자극음을 50–65 dB nHL 강도로 자극하여 정상 60개 귀의 100%에서 골도 cVEMP 반응을 관찰한 Miyamoto 등[12]의 연구나 특수 골도 진동기인 Mini-shaker (Brüel & Kjær, Naerum, Denmark)의 강력한 진동을 이용하여 95 dB nHL 이상으로 최대 자극 강도를 주어 정상 48개 귀의 100%에서 골도 cVEMP 반응을 관찰한 Na 등[16]의 연구와는 상당한 차이를 보였다. 한편, McNerney와 Burkard [14]는 B-71 골도 진동자를 통해 120 dB sound pressure level의 자극음으로도 기도나 골도 cVEMP가 발현되지 않은 일부 정상인 대상자는 연구에서 제외하였다고 기술하고 있다. 본 연구에서 골도 cVEMP의 발생률이 기존 보고보다 낮게 나온 이유는 60 dB nHL의 저강도로 골도 자극을 했기 때문이거나, B-71 골도 진동자가 골도 cVEMP의 측정에 효율적이지 못했기 때문으로 추정된다. 이에 본 저자들은 향후 70 dB nHL 이상의 고강도 골도 자극이나 효과적인 골도 진동자를 사용하는 후속 연구를 통해 골도 cVEMP의 낮은 발생률을 보완하는 방안을 모색하고 있다.

골도와 기도 cVEMP 검사의 지표를 비교함에 있어 자극음의 교차 청취(crossover hearing)에 대한 고려가 필요하다. 예를 들어, 기도 청력의 경우 500 Hz에서 약 40 dB의 이간 감약(interaural attenuation)이 있어, 본 연구에서처럼 90 dB nHL의 기도 자극음을 한쪽 귀에 줄 경우 50 dB nHL 크기로 골전도를 통해 반대쪽 귀에 도달하여 반대측 구형낭이 자극된다. 순음청력검사의 경우 반대쪽 귀에 잡음을 들려주어 차폐(masking)를 함으로써 청력 역치의 정확도를 높일 수 있으나, cVEMP 검사에서는 이를 해결할 만한 대안이 존재하지 않는다. 그리고, 골전도 진동기를 통한 음 자극의 이간 감약은 주파수에 따라 약간의 차이가 있으나 일반적으로 10 dB 이하이므로, 골도 자극은 모든 환자에서 양쪽 구형낭이 동시에 비슷한 강도로 자극된다고 볼 수 있다. McNerney와 Burkard [14]도 cVEMP 검사가 양측 귀의 동시 자극에 의한 상호작용이라는 것을 강조한 바 있다. 이러한 점들을 종합해보면 같은 크기의 자극음에 대해서도 양측 구형낭을 자극하는 정도의 차이가 있기 때문에 골도와 기도 cVEMP 검사의 결과가 다르게 나올 수 있다. 본 연구에서는 골도 자극음에 의한 cVEMP 반응은 기도 자극음에 비해 p13, n23 파형의 발생률이 낮고 진폭이 작으며, 잠복기, 파간 잠복기 차이, 양이간 진폭 비대칭성은 의미 있는 차이를 보이지 않았다. 다수의 연구들에 따르면, 본 연구의 결과와 비슷하게 정상인에서 골도 cVEMP의 지표들은 기도 cVEMP에서의 값과 대부분 동일하다고 한다[2,5,6,12-15]. Taylor 등[17]은 골도 및 기도 cVEMP 검사에서 본 연구와 달리 골도 자극일 때 양이간 진폭 비대칭성이 8.7%±6.2%, 기도 자극일 때 11.2%±10.7%로 골도 cVEMP에서 양이간 진폭 비대칭성이 유의하게 감소하는 경향을 보인다고 보고했다. 이에 따라 모든 cVEMP 임상 연구에서 자극음의 교차 청취를 염두에 두어야 하며, 검사 결과의 해석에 주의가 필요하다.

결 론

본 연구에서 비특이적인 어지럼증 환자와 정상인에서 골도와 기도 자극에 의한 cVEMP를 시행한 결과, 골도-기도 두 검사 간에 잠복기, 파간 잠복기 차이, 양이간 진폭 비대칭성은 별다른 차이가 없으나, 기도 자극에 의한 cVEMP 반응에 비해 골도 cVEMP 반응의 전체적인 발생률이 더 낮았고, p13–n23 파형의 진폭이 작았다. 이는 현재의 어지럼증 검사 상 골도 cVEMP를 시행하고 결과를 해석할 때 주의해야 함을 시사하며, 이러한 문제점을 해결하기 위하여 더 고강도의 골도 자극을 이용하거나, 기존의 진동자 이외에 골도 cVEMP에 특화된 특수한 골도 진동자가 필요할 것으로 생각된다.
