Abstract

Objective
The purpose of this study was to identify the diverse patterns of nystagmus and analyze their clinical significance in benign paroxysmal positional vertigo (BPPV) of the anterior semicircular canal.
Methods
Fifty-three patients diagnosed with anterior canal BPPV (AC-BPPV) were analyzed retrospectively. Patients were classified according to the presence or absence of the torsional component of the nystagmus and the direction of Dix-Hallpike test which induced the nystagmus. We compared the clinical characteristics and treatment outcomes among the different patient groups.
Results
There were 11 patients with unilateral down beat (DB) nystagmus, 11 patients with bilateral DB nystagmus, 14 patients with ipsilateral torsional down beat (TDB) nystagmus, 7 patients with contralateral TDB nystagmus, and 7 patients with bilateral TDB nystagmus. There were no differences between the unilateral and the bilateral DB groups in terms of the duration of nystagmus or vertigo and the number of treatment sessions. In addition, the ipsilateral TDB group showed no significant clinical difference compared to the contralateral or bilateral TDB group.
Conclusion
Various nystagmus patterns can be seen in AC-BPPV. There was no statistically significant difference in the clinical characteristics according to the different nystagmus patterns. This information may be helpful for clinicians in counseling and managing the patients with AC-BPPV.

서 론

양성 돌발성 두위현훈(benign paroxysmal positional vertigo, BPPV)은 회전성 어지럼을 일으키는 말초성 내이 질환 중 가장 높은 빈도를 보이는 단일 질환이다[1,2]. BPPV는 일반적으로 단일 반고리관을 침범하며, 후반고리관, 수평반고리관, 전반고리관 BPPV로 분류한다. 각 반고리관에 이환되는 빈도는 후반고리관 BPPV (posterior canal BPPV, PC-BPPV)가 80%–90%, 수평반고리관 BPPV가 15%–20%, 전반고리관 BPPV (anterior canal BPPV, AC-BPPV)가 1%–2%라고 알려져 있다[3]. 전반고리관은 아래쪽으로 공통각에 연결되며, 서 있는 상태에서 가장 위쪽에 위치하는 해부학적 특성 때문에 발생빈도가 적은 것으로 알려져 있다[4,5].

AC-BPPV의 진단 기준은 아직까지 후반고리관 및 수평반고리관 BPPV에 비해 명확하지 않다[6,7]. 일부 저자들은 고개를 돌리는 방향에 관계없이 회선성분을 가지는 하방안진이 나타나면 진단할 수 있다고 보고하고 있으나[8,9], 다른 연구에서는 회선성분이 없이 하방안진만 나타나는 경우를 AC-BPPV로 정의하고 있으며[10], 이외에도 다른 반고리관과 동시에 전반고리관에 BPPV가 생기거나 양측의 전반고리관에 BPPV가 생기는 등의 여러 경우에 따라 다양한 안진이 보일 수 있다고 한다[11]. 저자들은 본원의 AC-BPPV 환자들에게서 보인 여러 형태의 안진을 분류해보았으며, 그에 따른 임상적인 특징의 차이를 분석해보았다.

대상 및 방법

2011년 1월부터 2015년 12월까지 본원 이비인후과로 내원하여 BPPV로 진단받고 이석정복술을 시행한 1204명의 의무기록을 후향적으로 검토하였다. BPPV의 진단은 2008년 미국 이비인후과학회에서 제시한 진료 지침에 따라 체위성 검사를 통해 각 이환된 반고리관을 결정하였다[6]. PC-BPPV의 진단은 Dix-Hallpike 검사에서 60초 이내의 특징적인 회선 상향 안진을 보이는 경우 확진하였고, 수평반고리관 BPPV의 진단은 수평면에서 머리회전검사(supine head roll test)에서 전형적인 향지성 또는 원지성 방향의 방향전환성 두위안진을 확인함으로써 확진하였다. AC-BPPV의 진단은 회전성 어지럼을 호소한 환자에서 Dix-Hallpike 검사상 회선성분의 유무에 관계없이 하방안진이 관찰되고, 안진의 지속시간이 60초 미만인 경우로 정의하였으며, 병변의 방향은 회선안진의 방향으로 결정하였다. 회선안진이 관찰되지 않는 경우에는 straight-back head-hanging 검사를 통해 회선안진을 확인하여 병변의 방향을 결정하였다. 다발성 반고리관 BPPV (multiple canal BPPV)의 진단은 Dix-Hallpike 검사와 supine head roll test에서 각각 어지럼 증상과 함께 특징적인 안진이 나타나는 경우, 그 특징적인 안진에 해당하는 반고리관의 조합으로 진단하였다. 안진의 확인은 computerized video eye movement recorder (SLMED, Seoul, Korea)를 사용하였다. AC-BPPV로 진단한 모든 환자들에서 자세한 신경학적 진찰을 시행하여 중추신경계의 이상을 감별하였으며, 신경학적 이상이 의심되는 경우 뇌자기공명영상(뇌 magnetic resonance imaging)를 촬영하여 중추성 원인을 배제하였다. AC-BPPV의 치료는 앉은 자세에서 고개가 30º 이상 신전되도록 똑바로 눕고 30초에서 1분간 유지한 뒤 턱이 흉부 쪽으로 향하도록 고개를 굴곡시키고 30초에서 1분간 유지한 후 다시 앉은 자세를 취하는 Yacovino maneuver를 시행하였다[10].

AC-BPPV의 다양한 안진 양상을 서로 비교 분석하기 위해, Dix-Hallpike 검사에서 관찰된 안진의 종류에 따라 다음과 같이 환자군을 분류하였다.

(1) 하방안진이 일측에서만 보이는 군(unilateral downbeat, Uni-DB),
(2) 하방안진이 양측에서 보이는 군(bilateral downbeat, Bi-DB),
(3) 회선안진과 하방안진이 병변측에서 보이는 군(ipsilateral torsional downbeat, Ipsi-TDB),
(4) 회선안진과 하방안진이 병변 반대측 또는 양측에서 보이는 군(contralateral or bilateral torsional downbeat, Contra- or Bi-TDB),
(5) 비전형적인 안진을 보이는 군(atypical nystagmus).

환자군 (1)과 (2), 그리고 환자군 (3)과 (4) 사이에 치료의 횟수, 안진의 지속시간, 현훈의 지속기간에 있어서 통계적으로 의미 있는 차이가 있는지를 각각 분석하였고, 회선성분의 안진을 보인 환자군과 하방안진만을 보인 환자군을 비교하기 위해 환자군 (1)＋(2)와 (3)＋(4)에 대해서도 분석해보았다. 통계적 분석을 위해 PASW ver. 18.0 (SPSS Inc., Chicago, IL, USA)을 사용하여 Mann-Whitney U test를 시행하였으며, p값이 0.05 미만인 경우 통계적으로 유의한 것으로 정의하였다.

결 과

전체 BPPV 환자들 1,204명 중 PC-BPPV로 진단받은 환자는 573명(47.6%), 수평반고리관 BPPV로 진단받은 환자는 542명(45.0%), AC-BPPV로 진단받은 환자는 53명(4.4%), 다발성 반고리관 BPPV로 진단받은 환자는 36명(3.0%) 이였다(Figure 1). AC-BPPV로 진단된 환자들의 Dix-Hallpike 자세에서 안진을 분석해보았을 때, Uni-DB가 11명(20.8%), Bi-DB가 11명(20.8%)이었다. 회선성분이 동반된 하방안진이 보인 경우가 28명(52.8%)으로 가장 많았으며, 그 중에서 Ipsi-TDB가 14명(26.4%), Contra-TDB가 7명(13.2%), Bi-TDB가 7명(13.2%)이었다. 마지막으로 3명(5.7%)은 비전형적으로 일측에서 회선성분과 하방안진이 보였지만 반대측에서는 회선성분 없이 하방안진만 보였다(Table 1). 또한, 회선성분이 없이 하방안진만 보인 군에서 straight-back head-hanging검사를 했을 때 회선성분이 보인 경우는 22명 중 2명(9.1%)으로 나타났다. Dix-Hallpike 검사의 방향에 따른 안진이 다르게 보인 군들에서 안진의 지속시간, 치료의 횟수, 현훈의 지속기간에 대해 통계적으로 유의한 차이가 있는지 분석해보았다.

첫 번째로 Uni-DB군과 Bi-DB군을 비교를 했을 때, 안진의 지속시간, 치료의 횟수, 현훈의 지속기간은 두 군 사이에 유의한 차이가 없었고(p＞0.999, p=0.606, p=0.171), 두 번째로 Ipsi-TDB군과 Contra- or Bi-TDB군을 비교해보았을 때도 안진의 지속시간, 치료의 횟수, 현훈의 지속기간이 두 군간에 유의한 차이가 나타나지 않았다(p=0.777, p=0.094, p=0.910). 마지막으로, 하방안진만 보인 환자군(환자군 [1], [2])과 회선성분의 안진이 관찰된 환자군(환자군 [3], [4])의 비교에서도 안진의 지속시간, 치료의 횟수, 현훈의 지속시간이 유의한 차이를 보이지 않았다(p=0.132, p=0.183, p=0.109)(Table 2). AC-BPPV 환자들에서 Yacovino maneuver로 치료 후 PC-BPPV로 전환된 경우는 53명 중 5명(9.4%)로 나타났다.

고 찰

후반고리관 및 수평반고리관 BPPV에 대한 연구는 오랜 기간에 걸쳐 많이 보고된 반면, AC-BPPV에 대한 연구는 아직 많지 않다[7]. 이는 AC-BPPV의 빈도가 다른 두 아형에 비해 매우 적고, 그 진단기준이 명확하지 않기 때문이라고 생각된다. 본 연구에서 전체 BPPV 환자 중 AC-BPPV가 차지하는 비율은 4.4%로 기존 연구에서 나타난 1%–2% 보다 높은 비율을 차지했다. 이는 본 연구에서 AC-BPPV의 진단 기준을 회선성분의 유무와 관계없이 하방안진이 있을 때로 정의하여 다양한 안진을 포함하였기 때문으로 생각된다. 또한, 한 연구에서는 비디오안진검사를 이용해 기록한 안진을 반복적으로 조사한 결과, AC-BPPV가 전체 BPPV 중 21%에 해당하는 것으로 보고하기도 하여 AC-BPPV에 대한 이해가 중요해졌다[12].

일반적인 AC-BPPV에서 보이는 안진은 병변 반대측에서 Dix-Hallpike 검사를 시행할 때 반고리관 내 이석들의 움직임으로 팽대부릉정이 팽대부의 반대쪽으로 움직여 원지성 회선성분이 동반된 하방안진(Contra-TDB)이 나타나게 된다[13-16]. Contra-TDB이 일측에서만 보이는 것은 Dix-Hallpike 검사의 순서로 설명이 가능하다[12]. 만약 Dix-Hallpike 검사를 병변 반대측부터 시행하면, Contra-TDB이 관찰되고 검사과정에서 이석들이 공통각 방향으로 이동하여 치료가 되어, 이어서 병변측으로 Dix-Hallpike 검사를 시행할 때 안진이 관찰되지 않을 수 있다[12].

향지성 회선성분이 동반된 하방안진이 일측에서만 나타나는 것(Ipsi-TDB)은 안진의 피로현상과 관련이 있다[17,18]. 만약 Dix-Hallpike 검사를 병변측부터 시행하면, 반고리관 내 이석이 팽대부 방향으로의 움직임으로 인해 증상과 병변 방향으로의 회선성분과 함께 하방안진이 나타나고, 연속적으로 병변 반대측에서 Dix-Hallpike 검사를 시행하면 이전 검사로 인한 안진의 피로현상이 일어나 안진이 나타나지 않을 수 있다. 그 외에도 팽대부에서부터 이석이 움직일 때 해부학적으로 상승되는 부분에서 이석이 이동하지 못해 안진이 보이지 않을 수도 있으며[19], Dix-Hallpike 검사에서 머리의 신전 정도에 따라 다양해지는 전반고리관의 각도도 영향이 있다고 알려져 있다[12].

이론적으로 Contra-TDB이 전형적인 AC-BPPV의 안진이지만, 본 연구에서 AC-BPPV 환자들의 다양한 형태의 안진 중 회선성분이 동반된 하방안진이 나타나는 경우가 52.8%로 매우 많은 비율을 차지했다. 이에 반하여 하방안진만 보이는 경우(Uni-DB 및 Bi-DB)도 41.6%로 많은 비율을 차지했는데, 하방안진만 관찰되는 이유는 여러 가지로 설명할 수 있다. 첫 번째, 후반고리관은 시상면부터 56º 의 각도로 위치하지만, 전반고리관은 41º 의 각도로 위치하여 Dix-Hallpike 검사를 시행했을 때 PC-BPPV와 다르게 회선성분의 안진을 보이는 벡터의 크기가 작아 육안으로 관찰할 때 회선성분의 안진이 약하여 하방안진만 주로 관찰될 수도 있다는 보고가 있다[20]. 두 번째, Aw 등[21,22]은 일상 생활에서 이루어지는 머리 회전은 2 Hz 내외의 범위에서 이루어지며, 이 때 발생하는 전정안반사에서 수평안진과 수직 안진은 1에 가까운 것에 비해 회선안진은 0.75 정도로 작다고 보고하고 있다. 임상적으로 체위성 검사에서 이를 적용하면, AC-BPPV의 진단 시 회선성분의 안진이 잘 관찰되지 않고 주로 수직 하방안진만 관찰되는 것을 이해할 수 있다. 마지막으로 다발성 반고리관 BPPV의 한 형태에서도 하방안진만 관찰될 수 있다는 가설이 있다[11]. 만약 일측의 후반고리관과 전반고리관에 동시에 BPPV가 생겼을 때, 병변 반대측으로 Dix-Hallpike 검사를 하면 전반고리관의 이석들은 팽대부의 반대쪽으로 이동하게 되어 흥분성 신호가 유발되고 원지성 회선성분이 동반된 하방안진(Contra-TDB)이 나타나며, 이와 동시에 후반고리관의 이석들은 팽대부쪽으로 움직여 억제성 신호가 유발되어 향지성 회선성분이 동반된 하방안진(Ipsi-TDB)이 나타나게 된다. 이 때, 유발된 안진들의 회선성분은 서로 상쇄되고 결국 하방안진만 관찰될 수 있다.

안진 양상에 따른 임상 양상의 차이를 통계학적으로 분석해보았을 때, 첫 번째로 Uni-DB군과 Bi-DB군에서 치료의 횟수와 안진의 지속시간 및 현훈의 지속기간은 모두 통계학적으로 유의한 차이를 보이지 않았고 두 번째로 비교한 Ipsi-TDB군과 Contra- or Bi-TDB군, 마지막으로 비교한 DB군과 TDB군에서도 치료의 횟수와 안진의 지속시간 및 현훈의 지속기간은 통계학적으로 유의한 차이를 보이지 않았다. 이는 다양한 하방안진의 양상을 보이는 AC-BPPV의 환자군들 사이에 임상 양상의 차이가 없음을 의미하며, AC-BPPV의 진단 기준에서 다양한 안진 양상이 포함될 수 있음을 의미한다.

이전 연구에서 AC-BPPV와 PC-BPPV의 관계를 알아보기 위해 AC-BPPV 치료 후 PC-BPPV로 전환된 비율을 알아보았을 때 12%였으나, 본 연구에서는 9.4%로 앞선 연구와 유사한 비율을 보였다[23]. 또한, Dix-Hallpike 검사에서 회선성분의 안진이 나타나지 않은 경우 straight-back head-hanging 검사를 하더라도 회선성분이 나타난 경우는 22명 중 2명(9.1%)에 불과했다. 일반적으로 대다수 연구들은 AC-BPPV를 Dix-Hallpike 검사를 통해 회선성분이 동반된 하방안진이 보이면 진단하고 있다. 본 연구에서 AC-BPPV로 진단하여 분석한 다양한 안진들의 임상양상을 비교해 보았을 때, 회선성분없이 하방안진만 관찰된 경우가 상당히 많았으며, 전형적인 회선성분을 보인 군들과 비교해보았을 때도 유의한 차이를 보이지 않는 것으로 보아, 비특이적인 안진이 나타나더라도 중추성 원인이 감별되었다면 임상적으로 AC-BPPV 진단 하에 치료 및 경과관찰을 하는데 큰 무리는 없을 것으로 보인다.

결 론

AC-BPPV는 Dix-Hallpike검사에서 회선성분이 동반된 하방안진이 나타날 수도 있고 하방안진만 나타날 수도 있다. 또한 양측에서 모두 보이거나 일측에서만 관찰될 수 있어 다양한 형태의 안진으로 관찰된다. 본 연구에서 다양한 형태의 안진을 관찰할 수 있었고, 안진의 양상이 다른 군들 간에 치료의 횟수 및 치료 기간에 있어 통계학적으로 유의한 차이를 보이지 않았다. 이러한 결과는 추후 AC-BPPV가 의심되는 환자들의 진단 및 치료에 있어 유용한 정보가 될 수 있을 것으로 기대된다.
